经阴道与腹腔镜子宫全切术治疗子宫良性疾病的临床效果对比研究

摘　　要：目的对比经阴道与腹腔镜子宫全切术治疗子宫良性疾病的临床效果。方法选取本院2015年1月至2017年6月收治的子宫良性疾病患者200例,按照随机数字表法分为阴式组和腹腔镜组各100例。比较两组患者的手术时间、术中出血量、术后肛门排气时间、术后住院时间、术后及远期并发症发生率、手术及麻醉费用,通过电话对患者进行随访,比较远期并发症发生率。结果阴式组患者的手术时间、术中出血量、术后肛门排气时间、术后住院时间、术后及远期并发症发生率、麻醉费用、手术费用与腹腔镜组相比,差异均有统计学意义(P<0.05)。术后6个月对两组患者进行随访,末次随访时间为2017年12月,两组患者远期并发症发生率差异无统计学意义(P>0.05)。结论与腹腔镜子宫全切术相比,阴式子宫全切术在手术指标以及术后安全性方面具有明显的优势,但是在术前需要严格评估手术危险因素、把握手术指征,以减少并发症、提高手术安全性。Objective To compare the clinical effect of vaginal total hysterectomy （VTH） and lap aroscopic total hysterectomy（LTH） for patients with benign uterine diseases. Methods A total of 200 patients with benign uterine diseases from January 2015 to June 2017 of our hospital were selected and divided randomly into group VTH and group LTH. The duration of operation, intr aoperative blood loss, postoperative anus exhausting time, postoperative hospital stay, rates of postoperative and long term complications and cost of surgery and anesthesia were compared be tween the two groups. Results The differences of duration of operation, intraoperative blood loss, postoperative anus exhausting time, postoperative hospital stay, rates of postoperative corn plications and cost of surgery and anesthesia between the two groups were statistically significant （P〈0.05）. The rates of long term complications after 6 months of the surgery of the two groups were similar, the difference was not statistically significant （P〉0.05）. Conclusions The opera tion related index and postoperative safety of the VTH were significantly better than the LTH. It is necessary to strictly estimate the preoperative risk factors and grasp decrease the rate of complications and raise the operation safety.
