摘 要:目的探讨2型糖尿病坐骨神经病变的超声显像特征。方法选取2017年2月~2018年2月在四川省内江市第一人民医院治疗的糖尿病伴坐骨神经病变患者70例(A组),糖尿病无坐骨神经病变患者70例(B组),同时选取健康者70名作为对照(C组)。采用高频超声观察各组坐骨神经的特征。结果 A组神经被膜增厚粗糙比例明显高于B组和C组(P<0.05);三组神经束膜增厚粗糙、神经内部回声减低比例比较,差异无统计学意义(P>0.05);A组坐骨神经前后径(D1)、横径(D2)和横切面积(CSA)明显高于B组和C组(P<0.05);B组和C组坐骨神经D1、D2和CSA比较,差异无统计学意义(P>0.05);A组男性患者D1、D2和CSA明显高于女性患者(P<0.05)。结论高频超声可清晰显示坐骨神经参数,可为2型糖尿病坐骨神经病变诊断及治疗提供客观依据。Objective To investigate ultrasonography characteristics of type 2 diabetes mellitus with sciatic neuropathy.Methods From February 2017 to February 2018, in the First People's Hospital of Neijiang City, 70 cases of diabetic patients with sciatic neuropathy(group A), 70 cases of diabetic patients without sciatic neuropathy(group B) were selected, and 70 healthy persons were selected as control(group C) at the same time. The characteristics of sciatic nerve in each group were observed by high frequency ultrasound. Results The proportion of nerve capsule thickening and rough in group A was significantly higher than those in group B and group C(P〈0.05); the proportion of thickening and rough of the perineal membrane, the reduction of the echo of the inner were compared among three groups, and the differences were not statistically significant(P〈0.05). The anteroposterior diameter(D1), transverse diameter(D2) and cross sectional area(CSA) of sciatic nerve in the group A were significantly higher than those in the group B and group C(P〈0.05); the D1, D2 and CSA of sciatic nerve in group B and group C were compared, the differences were not statistically significant(P〈0.05). In group A, the D1, D2 and CSA of male patients were significantly higher than those of female patients(P〈0.05). Conclusion High frequency ultrasound can clearly display the parameters of sciatic nerve, and it can provide objective evidence for the diagnosis and treatment of type 2 diabetic sciatic neuropathy.
