摘  要:目的探讨矽肺患者血清中的铜蓝蛋白(ceruloplasmin,CP)含量和单胺氧化酶(monoamine oxidase,MAO)、血管紧张素转化酶(angiotensin converting enzyme,ACE)的活力,及其三者与矽肺患者肺部病变程度的相关性。方法以100名矽肺患者为病例组,50名健康人作为对照组。分别采用免疫透射比浊法、连续监测法、呋喃丙烯酰苯丙氨酸甘氨酰甘氨酸(furan acryloyl phenylalanine glycylglycine,FAPGG)底物法对两组人群血清中的CP含量及MAO、ACE活力进行检测;对病例组进行血气分析、肺通气功能检测,并对结果进行比较和相关性分析。结果病例组CP含量高于对照组(P<0.05),两组间的MAO、ACE活力比较,差异无统计学意义(P>0.05)。矽肺患者的CP含量、MAO、ACE活力与患者年龄、粉尘接触时间无相关关系(P>0.05);矽肺叁期患者血清中的CP含量高于壹期患者(P<0.05),矽肺不同分期患者间血清的MAO、ACE活力比较,差异无统计学意义(P>0.05);矽肺患者氧分压正常与异常组间CP含量、MAO、ACE活力比较,差异无统计学意义(P>0.05);按通气功能分级,中度、重度通气功能障碍组患者血清的CP含量高于正常组(P<0.05),MAO、ACE活力组间比较,差异无统计学意义(P>0.05);按小气道功能分级,重度异常组患者血清里的CP含量、MAO活力高于正常组(P<0.05),ACE活力组间比较,差异无统计学意义(P>0.05)。结论矽肺患者血清中的CP能在一定程度上反映矽肺患者的肺部病变程度;MAO仅在小气道功能重度异常者MAO中出现活力差异,可能参与了疾病早期小气道的病变过程;ACE与矽肺肺部病变程度的相关性仍需进一步研究。Objective To investigate the level of ceruloplasmin(CP) and the activity of monoamine oxidase(MAO),angiotensin converting enzyme(ACE) in the serum of patients with silicosis, and to explore the relationship between them and the degree of pulmonary lesions in silicosis patients. Methods One hundred patients with silicosis were chosen as the case group; Fifty healthy people were selected as the control group. The level or the activity of CP, MAO and ACE in serum of the two groups were detected by immune transmission turbidity method, continuous monitoring method and FPAGG substrate method respectively. Blood gas analysis and pulmonary ventilation function were performed in the case group, and the results were compared and analyzed for the correlativity. Results The level of CP in the case group was higher than that in the control group(P〈0.05); There was no significant difference in the activity of MAO and ACE between the two groups(P〉0.05). The level or the activity of CP, MAO and ACE in silicosis patients were not related to the age of the patients and the exposure time of the dust(P〉0.05). The serum levels of CP in the patients with silicosis in stage Ⅲ was higher than that in stage Ⅰ(P〈0.05); There was no significant difference in the activity of MAO and ACE among patients with different stages of silicosis(P〉0.05).There was no significant difference of CP, MAO and ACE between the two groups with normal or abnormal oxygen partial pressure in silicosis patients(P〉0.05). According to the classification of ventilation function, the serum levels of CP in the patients with moderate or severe ventilation dysfunction were higher than those in the normal group(P〈0.05),but there was no significant difference in the activity of MAO and ACE among the groups(P〉0.05); According to the small airway function classification, the serum levels of CP and the activity of MAO in the patients with severe abnormalities were higher than those in the normal group(P〈0.0
