6例主肺动脉窗合并主动脉弓离断的超声心动图及心脏造影诊断价值

摘　　要：目的总结超声心动图及右心造影、主动脉造影在先天性心脏病主肺动脉窗合并主动脉弓离断等复杂畸形的诊断价值及经验，提高临床诊断的准确率。方法回顾性分析北京安贞医院应用超声心动图对6例小儿主肺动脉窗合并主动脉弓离断心脏解剖及血流动力学异常进行检测，并经右心造影及主动脉造影证实，结合临床总结主肺动脉窗合并主动脉弓离断影像特征与临床特点。结果主肺动脉窗合并主动脉弓离断超声心动图表现特征右肺动脉起自升主动脉后壁，主、肺动脉间隔回声缺失，主肺动脉间隔双向低速分流信号，动脉导管未闭及动脉导管处右向左分流信号，主动脉弓离断，右心增大，右心室壁肥厚。右心造影特征，右心室显影后，肺动脉显影，随后主动脉显影。结论超声心动图结合心脏造影检查在诊断复杂先天性心脏病主肺动脉窗合并主动脉弓离断诊断的准确率意义重大。Objective To summarize the diagnostic value and experience of ultrasound, right cardioangiography and aortography in the diagnosis of congenital heart diseases such as the aorto-pulmonary window combined with interrupted aortic arch, and to improve the accuracy of clinical diagnosis. Methods The cardiac anatomy and hemodynamic abnormalities in 6 children with the aorto-pulmonary window combined with interrupted aortic arch by ultrasound, and confirmed by right cardioangiography and aortography were retrospectively analyzed, and the image characteristics and clinical features of this diseases were summarized. Results Echocardiography features showed： The right pulmonary artery originates from the posterior wall of the ascending aorta. The echoes of the aortopulmonary septa was defected, and the bidirectional low-speed shunt signal of aorto-pulmonary septa. Patent ductus arteriosus and the right to left shunt signals of ductus arteriosus. The interruption of aortic arch. right heart enlargement and right ventricular wall hypertrophy. Right cardioangiography showed that after the development of the right ventricle, the pulmonary arteries were developed and then the aorta was developed. Ascending aorta angiography showed that the contrast agent could not enter the descending aorta. Conclusion Ultrasound combined with cardiac angiography is of great significance in the diagnosis of complex aortic heart disease like aorto-pulmonary window combined with interrupted aortic arch.
