美托洛尔结合曲美他嗪对冠心病心力衰竭患者临床疗效及血浆脑钠肽的影响研究

摘　　要：目的 探析冠心病（CHD）心力衰竭（心衰）患者应用美托洛尔联合曲美他嗪治疗的效果及血浆脑钠肽（BNP）改善情况.方法 将我院心内科2015年5月至2017年10月收治的80例CHD心衰患者纳入研究中,通过数字表法随机分成观察组和对照组,各40例;两组均给予常规专科治疗,同时对照组应用美托洛尔,观察组联用美托洛尔、曲美他嗪,连续用药12周,对比分析两组疗效,并测定治疗前后血浆BNP及心功能指标改善情况.结果 观察组治疗总有效率为97.50％,对照组为82.50％,相比差异有统计意义（P＜0.05）;治疗后两组患者的血浆BNP、HR、LVEDD、LVESD及LVEF等指标水平均有改善,但观察组改善效果优于对照组（P＜0.05）;两组治疗中均未出现明显不良反应.结论 在CHD心衰患者治疗中,联用美托洛尔和曲美他嗪,可有效提升疗效,改善患者血浆BNP水平和心功能,且安全性较高,值得临床应用.Objective To explore the effect of the treatment of coronary heart disease （CHD） heart failure patients and the improvement of the plasma brain natriuretic peptide （BNP）. Methods In the study of 80 cases of CHD heart failure patients admitted to our hospital from May 2015 to October 2017, a total of 40 cases were ran- domly divided into observation group and comparison group by digital table method. Two groups were given rou- tine specialist treatment, while control group was given Metoprolol, observation group was added Trimetazidine based on Metoprolol. After 12 weeks, the curative effect, the determination of plasma BNP before and after the treatment, systolic blood pressure （SBP） and heart rate （HR） were compared between two groups. Results The total effective rate of observation group was 97.50%, compared with 82.50% in the comparison group, which was statistically significant （P〈0.05）. The plasma BNP and cardiac function indicators （HR, LVEDD, LVESD, LVEF） of the two groups were improved, but the improvement of observation group was better than that of the comparison group （P〈0.05）. There was no obvious adverse reaction in both groups. Conclusion In the treatment of patients with CHD heart failure, the combination of Metoprolol and Trimetazidine can effectively improve the efficacy of heart function, reduce the plasma BNP and have high safety, which is worth clinical application.
