影响过敏性紫癜患儿肾脏损伤的相关因素分析

摘　　要：目的通过总结过敏性紫癜(HSP)患儿的临床特点及相关实验室检查结果,分析HSP患儿发生肾脏损伤的相关危险因素。方法收集兰州大学第一医院确诊为HSP的130例患儿的临床资料,根据有无肾脏损伤分为HSP组(104例)和紫癜性肾炎(HSPN)组(26例),比较2组的临床症状及感染、免疫功能、凝血、尿早期肾损伤指标,并分析影响肾脏损伤的相关危险因素。结果 2组在腹痛症状,免疫球蛋白G(Ig G)、补体C4降低,微量白蛋白(ALB)、N-乙酰-β-D-葡萄糖苷(NAG)酶升高等5个方面比较,差异均有统计学意义(P<0.05或P<0.01);经非条件Logistic回归分析,Ig G降低、ALB升高是HSP患儿发生肾脏损伤的相关危险因素。结论 Ig G降低、ALB升高是HSP患儿发生肾脏损伤的相关危险因素,这对判断HSP患儿的预后有一定的指导意义,对于有上述因素的HSP患儿更应加强随访。Objective To analyze the related risk factors of renal injury in children with anaphylactoid purpura by summarizing the clinical features and relevant laboratory tests. Methods First,Collect the clinical data of130 children diagnosed as anaphylactoid purpura from the first hospital of Lanzhou university. Then,according to whether they have renal injury,26 cases were diagnosed as purpura nephritis,104 cases as anaphylactoid purpura.The clinical symptoms,immune function,infection indexes,blood coagulation function and early renal injury index of two groups were compared to analyze the related risk factors of renal injury. Results The comparisons between two groups in abdominal pain,the reduction of immunoglobulin Ig G and complement C4,as well as the growth of urine albumin and NAG enzyme had statistical significant differences（ P〈0.05 or P〈0.01）; the unconditioned Logistic regression analysis results suggested that the reduction of immunoglobulin Ig G and the growth of urine albumin are risk factors of renal injury.Conclusion The reduction of immunoglobulin Ig G and the growth of urine albumin are risk factors of renal injury,which has a guiding significance for judging the prognosis of children with anaphylactoid purpura.Therefore,the followed up to the anaphylactoid purpura chilldren with these above factors should be enforced.
