耐多药结核病患者护理管理效果的影响因素研究

摘　　要：目的回顾耐多药结核病患者的护理管理过程,探讨护理相关因素对耐多药结核病患者治疗的影响,为今后更好地护理管理耐多药结核病患者提供借鉴。方法分析实施项目过程中41例耐多药结核病患者治愈、治疗失败、丢失、死亡的护理相关原因,总结耐多药结核病患者化疗过程中的护理重点和难点。结果参与全程护理管理的41例耐多药结核病患者治愈19例,治疗失败11例,丢失6例,死亡5例,分别占46.3%、26.8%、14.6%和12.2%。41例耐多药患者抗结核治疗护理依从性大于95%者25例,占61%,依从性小于95%者16例,占39%;护理依从性大于95%者中治愈17例,治愈率68%,依从性小于95%者中治愈2例,治愈率12.5%(X^2=12.09,p<0.01);影响依从性的因素分别为心理因素16例(X^2=33.53,p<0.01)影响力100%;用药不良反应8例(X^2=9.53,p<0.01),影响力50%;家庭问题8例(X^2=9.53,p<0.01),影响力50%。结论护理依从性好的耐多药结核病患者治愈率高,耐多药结核病患者护理依从性主要受患者心理问题、家庭支持不力及药物不良反应影响。Objective Review the process of nursing management of MDR-TB patients to explore the influence of nursing related factors on the treatment of MDR-TB patients, and to provide reference for better nursing management of MDR-TB patients in the future.Method Forty-one MDR-TB patients were studied, and analyzed for their healing, failure, loss, and mortality related nursing causes.Result Among Forty-one patients with MDR-TB, nineteen cases were cured, eleven cases were cured and six cases were lost and five cases died（46.3%, 26.8%, 14.6%, 12.2%）. The influencing factors of nursing compliance were psychological factors （16/16, X2= 33.53, p〈0.01 ）, drug side effects （8/16, X^2=9.53, p〈0.01 ）and family problems （8/16, X^2=9.53, p〈0.01 ）.Conclusion The cure rate of MDR-TB patients with good nursing compliance is high, in addition, nursing compliance is mainly affected by patients＂ psychological problems, poor family support and adverse drug reactions.
