摘  要:目的探讨血清抗苗勒管激素与卵巢储备功能关系以及临床应用价值。方法回顾性分析2016年3月到2018年2月我科接受辅助生殖技术治疗的132例患者并将其分为4组。A组:卵巢功能正常组(37例);B组:多囊卵巢综合征(PCOS)组(35例);C组:卵巢功能减退(DOR)组(34例);D组:卵巢早衰(POF)组(26例)。检测上述4组患者血清基础抗苗勒管激素(AMH)、卵泡刺激素(FSH)、雌二醇(E2)、黄体生成激素(LH)、孕酮(P)、催乳激素(PRL)、睾酮(T)及窦卵泡数(AFC)并做统计学分析。结果抗苗勒管激素(AMH)在上述4组中以多囊卵巢综合征(PCOS)组最高、卵巢早衰(POF)组最低,且4组间比较均具有统计学差异(P≤0.05)。抗苗勒管激素(AMH)与性激素比较:抗苗勒管激素(AMH)与卵泡刺激素(FSH)、黄体生成激素(LH)成负相关但与雌二醇(E2)、孕酮(P)以及窦卵泡数(AFC)成正相关。结论抗苗勒管激素(AMH)是女性生殖功能评估的重要指标,其血清水平与卵巢储备功能密切相关,结合窦卵泡数及基础内分泌激素检测能更准确地评估卵巢储备功能。Objective To explore the relationship between serum anti-Mullerian hormone(AMH) and ovarian reserve function and clinical application value. Methods 132 patients who received assisted reproductive technology in our department were collected from March 2016 to Febuary 2018 and divided them into four groups. Group A:normal ovarian function group(37 cases);Group B:polycystic ovary syndrome(PCOS) group(35 cases);Group C:ovarian dysfunction(DOR) group(34 cases);Group D:premature ovarian failure(POF) group(26 cases). Serum basal AMH,follicle stimulating hormone(FSH),estradiol(E2),luteinizing hormone(LH),progesterone(P),prolactin(PRL),testosterone(T) and number of antral follicles(AFC) were detected and analyzed.Results AMH was the highest in the PCOS group and the lowest in the POF group in the above four groups,and there was a statistically significant difference among the four groups(P≤0.05). The comparison between AMH and sex hormones:AMH was negatively correlated with FSH and LH,but positively correlated with E2,P and AFC. Conclusion AMH is an important indicator of female reproductive function assessment. The serum level of AMH is closely related to the ovarian reserve function. Combined with AFC and basal endocrine hormone tests,ovarian reserve function can be evaluated more accurately.
