摘 要:目的探讨双能量盆腔CT虚拟平扫(VNC)与常规平扫(CNC)的图像质量差异。方法收集2017年7月~2018年1月单盆腔增强扫描的60例患者,行常规平扫和实质期双能量扫描。测量虚拟平扫及常规平扫图像上臀大肌及膀胱的平均CT值、臀大肌SNR、臀大肌-膀胱对比噪声比(CNR)及辐射剂量,并记录图像质量主观评分。结果虚拟平扫所测膀胱SNR及图像质量评分与常规平扫比较,差异无统计学意义(P>0.05);所测膀胱CT值、臀大肌CT值、臀大肌SNR、CNR比较,差异有统计学意义(P<0.05);双能量盆腔直接增强扫描所测辐射剂量低于常规盆腔增强扫描,差异有统计学意义(P<0.05)。结论双能量盆腔CT虚拟平扫的图像质量跟常规平扫的图像质量相比无差别,在去除节育环等金属伪影方面甚至优于常规平扫。因此虚拟平扫可替代常规平扫并且可以减少1次辐射剂量。Objective To investigate the difference of image quality between dual-energy pelvic CT virtual plain scan (VNC) and conventional plain scan(CNC). Methods 60 patients undergoing single pelvic enhanced scan between July 2017 and January 2018 were collected. Conventional plain scans and dual-energy scans in nephrographic phase were carried out. The plain CT value of gluteus maximus and bladder, SNR, gluteus maximus-bladder contrast noise ratio (CNR), and radiation dose on virtual plain scan and conventional plain scan images were measured. The subjective scores of image quality were recorded. Results There were no statistically significant differences in the urinary bladder SNR and image quality scores measured by the virtual plain scan compared with the conventional plain scan(P〉0.05); the measured CT value of the bladder, the CT value of the gluteus maximus, the SNR and the CNR of the gluteus maximus were all statistically different(P〈0.05); the measured radiation doses of dual-energy pelvic direct enhanced scan were lower than those in conventional pelvic enhanced scans, the difference was statistically significant(P〈0.05). Conclusion The image quality of dual-energy pelvic CT virtual plain scan has no difference compared with the image quality of conventional plain scan. It is even superior to conventional plain scan in removing metal artifacts such as birth control rings. Therefore, the virtual plain scan can replace the conventional plain scan and can reduce the radiation dose once.
