摘　　要：目的探讨应用后路经椎管腰椎椎间融合术(posterior lumbar interbody fusion,PLIF)治疗老年退行性腰椎管狭窄症的疗效。方法 2011年6月—2014年6月采用PLIF术治疗80例老年退行性腰椎管狭窄症,男34例,女46例;年龄56～76岁,平均63.5岁;采用腰椎功能障碍指数(oswestry disability index,ODI)及日本骨科协会评估治疗分数(Japanese orthopaedic association,JOA)评定手术患者术后恢复效果。结果 80例均获得随访,时间16～27个月,平均20个月;患者手术前及手术后MRI扫描腰椎椎管前后径改变差异有统计学意义(t=8.062, P<0.05),手术前与手术后的ODI分值改变差异有统计学意义(P<0.05),手术前与手术后的JOA评定分值改变差异有统计学意义(P<0.05),患者的平均改善率82.32%。结论 PLIF手术治疗老年退行性腰椎管狭窄症具有明显优势,患者恢复较快,可取得较为满意的临床效果。Objective To investigate the efficacy of posterior lumbar interbody fusion （PLIF） in the treatment of elderly degenerative lumbar spinal stenosis. Methods From June 2011 to June 2014, 80 patients with degenerative lumbar spinal stenosis were treated with PLIF. There were 34 males and 46 females with an average age of 63.5 years （range, 56-76 years）. The lumbar dysfunction index （oswestry） was used. The disability index （ODI） and the Japanese orthopaedic association （JOA） assessed the postoperative recovry of surgical patients. Results All the 80 patients were followed up for 16 to 27 months with an average of 20 months. The anterior and posterior diameter changes of the lumbar spine were statistically significant （P〈0.05） before and after surgry. The change of ODI score after operation was statistically significant （P〈0.05）. The change of JOA scores before and after surgry was statistically significant （P〈 0.05）, and the average improvement of patients was 82.32%. Conclusion PLIF surgery has obvious advantages in the treatment of senile degenerative lumbar spinal stenosis. The patient recovers quickly and can obtain satisfactry clinical results.
