剖宫产术后子宫切口部位妊娠保守治疗的临床分析

摘　　要：目的分析不同保守治疗方案在剖宫产术后切口部位妊娠患者中的应用效果。方法择取2016年1月—2018年3月该院收治的68例接受保守治疗的剖宫产术后切口部位妊娠患者进行分组研究,根据治疗方案不同,将其分为观察组(联合应用米非司酮、甲氨蝶呤)以及对照组(应用甲氨蝶呤单药治疗),各34例。比较两组总有效率、阴道停止出血时间、康复出院时间。结果观察组患者的总有效率达94.12%,明显高于对照组73.53%(χ~2=5.314,P=0.021)。观察组阴道停止出血时间、康复出院时间分别为(5.54±1.19)d、(11.28±4.19)d,均明显短于对照组(t=7.206、3.282,P<0.05);1个疗程后,观察组HCG水平为(324.52±15.18)U/L,显著低于对照组(721.32±25.47)U/L(t=78.033,P<0.05)。结论联合应用米非司酮、甲氨蝶呤治疗剖宫产术后切口部位妊娠的临床疗效好、康复时间短,值得推广及应用。Objective To analyze the application effect of conservative therapy of uterus incision pregnancy after the cesarean section surgery. Methods 68 cases of patients with uterus incision pregnancy after the cesarean section surgery admitted and treated in our hospital from January 2016 to March 2018 were selected and divided into two groups according to different treatment plans, with 34 cases in each, the observation group were treated with mifepristone combined with methotrexate, while the control group were treated with methotrexate single drug, and the total effective rate, vaginal bleeding stopping time and discharged time after recovery were compared between the two groups.Results The total effective rate in the observation group was obviously higher than that in the control group[（94.12% vs73.53%（χ2=5.314,P=0.021）], and the vaginal bleeding stopping time and discharged time after recovery in the observation group were respectively（5.54±1.19） d,（11.28±4.19） d, which were obviously shorter than those in the control group,t= 7.206, 3.282, P0.05）, after one treatment course, the HCG level in the observation group was obviously lower than that in the control group[（324.52±15.18）U/L vs（721.32±25.47）U/L（t=78.033, P0.05） ]. Conclusion The mifepristone combined with methotrexate in treatment of uterus incision pregnancy after the cesarean section surgery has a better curative effect and short recovery time, and it is worth promotion and application.
