摘 要:正常人血压应低于140/90mmHg(收缩压/舒张压),高于此标准即为高血压。高血压病容易并发冠状动脉粥样硬化性心脏病(简称,冠心病)、脑血管意外、肾功能衰竭等,是最常见的慢性病。目前对血压的监测多采用柯氏音等间歇测量法,这种方法只能得到人体某一时刻的血压值,很难进行实时监测。为了对高血压患者的血压进行实时监测,市面上出现了可以连续测量血压的方法和设备。连续性血压测量可以一天24h对血压进行监控,不需要医生的参与,对高血压病患者长期服用降压药的效果起到良好的监督作用,为临床诊断及后期治疗提供了更加充分的依据。本文研究了基于ECG和PPG信号的无袖带连续性血压测量方法的可操作性和测量准确性。该方法结合了基于大量训练数据的大数据分析方法和基于个体标定数据的个性化预测分析方法。其中,大数据分析方法通过远程服务器实现,然后将结果发送到智能手机App上进行显示;个体的血压计算可以在手机上实时离线完成。通过对91名受试者进行测试,测量结果满足标准AAMI SP10和IEEE 1708的要求,研究结果表明该方法可准确预测收缩压(SBP)和舒张压(DBP),实现了血压的实时监测,且操作简便。Normal people's blood pressure should be lower than 140/90mmHg (systolic/diastolic), that higher than this standard is considered as hypertension. Hypertension can be easily associated with coronary heart disease, cerebrovascular accident, and renal failure. It is one of the most common chronic diseases. At present, the monitoring of blood pressure is often performed by intermittent measurement such as Korotkof sounds measurement. This method can only obtain the blood pressure value of the human body at a certain time, and it is difcult to perform real-time monitoring. In order to monitor the blood pressure of hypertensive patients in real time, methods and devices that can continuously measure blood pressure emerge in the market. Continuous blood pressure measurement can monitor blood pressure 24 hours a day without the participation of doctors. It exerts good supervision efects on patients’ long-term use of antihypertensive drugs, providing a more sufcient basis for clinical diagnosis and treatment later. This paper studies the practicability and measurement accuracy of the sleeveless continuous blood pressure measurement method based on ECG and PPG signals. The method combines a big data analysis (based on a large amount of training data) and a personalized prediction analysis (based on individual calibration data). The big data analysis is implemented via a remote server, with the results sent to the smartphone App for display. the calculation on individual blood pressure can be completed of-line and in real time on the mobile phone. Via a test on 91 subjects, the measurement results meet the requirements of the AAMI SP 10 Standard and IEEE 1708 Standard. The results show that the method can accurately predict systolic blood pressure (SBP) and diastolic blood pressure (DBP), as well as realize real-time monitoring of blood pressure, which is also friendly to operate.
