加味新伤药外敷对胫骨平台闭合性骨折早期疼痛肿胀的疗效和护理观察

摘　　要：目的:观察护理配合加味新伤药外敷对胫骨平台闭合性骨折早期疼痛和肿胀的疗效观察。方法:选择我院收治的55例胫骨平台闭合性骨折的患者随机分为对照组(n=27)和观察组(n=28)例,对照组采用常规治疗护理措施,治疗组在常规治疗护理的基础上予以加味新伤药外敷,同时结合中医理论进行辨证护理;观察记录两组患者治疗前及治疗1、12、24、48、72小时患肢VAS评分、肿胀度、肿胀值;并记录两组患者患肢一周内出现骨筋膜室综合征、张力性水疱及皮肤皱褶的情况和治疗过程中并发症发生情况。结果:两组患者治疗方法与时间在疼痛VAS评分、肿胀度、肿胀值上存在交互作用(P≤0. 05);治疗方法在VAS评分、肿胀度、肿胀值上主效应显著(P≤0. 05);时间在VAS评分、肿胀度、肿胀值上主效应显著(P≤0. 05)。其中干预12小时后观察组患者疼痛VAS评分较对照组明显降低(P≤0. 05),72小时后患者疼痛VAS评分无差异(P>0. 05);干预24小时后观察组患者肿胀度较对照组明显降低(P≤0. 05);观察组患者患肢一周内出现OCS、张力性水疱发生率明显低于对照组(P≤0. 05);观察组患肢在一周内即出现明显消退而出现皮肤皱褶与对照组相比均明显加快(P≤0. 05);并且治疗后两组患者血常规、肝功能、肾功能均无因药物使用所造成的不良反应发生。结论:护理配合加味新伤药外敷对胫骨平台闭合性骨折早期疼痛和肿胀具有较好的作用。Objective： To investigate the effect of nursing combined with Modified Xinshang Medicine external application on early pain and swelling for closed fracture of tibial plateau. Methods： 55 cases of closed fracture of tibial plateau in our hospital were randomly divided into controlled group（ n = 27） and observation group（ n = 28）. The controlled group was treated with routine treatment. The treatment group was treated with Modified Xinshang Medicine on the basis of conventional treatment and nursing,and combined with the theory of traditional Chinese medicine; The VAS score,swelling and swelling of the two groups of patients with limb pain before and 1,12,24,48,72 hours were recorded,and the occurrence of OCS,tension blisters and skin folds in the two groups of patients and the complications during the treatment were recorded. Results： There was a interaction between the two groups of treatment methods and time on the pain VAS score,swelling degree and swelling value（ P ≤0. 05）,and the main effect was significant（ P≤0. 05） on the VAS score,swelling degree and swelling value,and the time was significant（ P≤0. 05） in the VAS score,swelling degree and swelling value. After the intervention of 12 hours,the pain VAS score of the patients in the observation group was significantly lower than that in the controlled group（ P≤0. 05）,and the pain VAS score of the patients after 72 h was no difference（ P≤0. 05）,and the swelling degree of the patients in the observation group was significantly lower than that of the controlled group after 24 hours intervention（ P≤0. 05）,and the incidence of OCS and tension blisters in the patients in the observation group was significantly lower than that in the observation group.
