基于DRGs的专科医疗机构绩效评价指标浅析

摘　　要：DRGs是国家医疗服务体制改革中引进的重要付费方式和管理手段,基于DRGs的医疗服务绩效评价指标目前缺乏在中医能力方面的评价指标;此外,"DRGs组数"和"低风险死亡率"对评价医疗机构诊疗范围和医疗质量存在一定的缺陷。因此,尝试构建"核心DRGs组数""诊疗广度指数""中医治疗指数""中药饮片使用指数""医院感染指数""并发症发生指数""非计划二次手术指数"指标,对基于DRGs的绩效评价指标进行补充和完善。同时,通过对某医院两病区同一DRGs病组的数据分析和思考,阐明了监测和更新DRGs分组器的必要性以及正确选择和编码主要诊断的重要性;此外,还指出了使用DRGs指标进行绩效评价管理时应采用合适的统计单元。DRGs is an important payment method and management method introduced in the reform of national medical service system.At present,DRGs-based evaluation index of medical service performance lacks the evaluation index of TCM ability.In addition,the “DRGs group number” and “low risk mortality rate” have certain defects in the evaluation of medical treatment scope and medical quality in medical institutions.Therefore,we try to build the indicators of “core DRGs group number”,“diagnosis and treatment breadth index”,“Chinese medicine treatment index”,“TCM slice use index”,“hospital infection index”,“complication occurrence index” and “non-planned secondary surgery index” to supplement and improve the indicators of performance evaluation based on DRGs.Simultaneously,through the data analysis and thinking of the same DRGs disease group in two wards of a hospital,the necessity of monitoring and updating the DRGs pacemaker as well as the importance of correct selection and coding of main diagnosis were clarified.In addition,it is pointed out that appropriate statistical units should be used in performance evaluation management with DRGs.
