复方苦参注射液联合多西他赛及奈达铂治疗中晚期宫颈癌临床研究

摘　　要：目的探讨复方苦参注射液联合多西他赛、奈达铂治疗中晚期宫颈癌的疗效及对免疫功能的影响。方法选取2015年1月至2017年1月收治的中晚期宫颈癌患者61例,采用随机数字表法分为对照组(31例)和观察组(30例)。在常规放射治疗(简称放疗)基础上,对照组化学治疗(简称化疗)方案采用多西他赛注射液+奈达铂,治疗组在对照组治疗基础上辅助静脉滴注复方苦参注射液。两组均以21 d为1个疗程,连续治疗2个疗程。结果治疗后,观察组总有效率和临床获益率分别为66. 67%和90. 00%,显著高于对照组的38. 71%和61. 29%(P <0. 05);观察组生存质量总改善率为90. 00%,显著高于对照组的67. 74%(P <0. 05);治疗后,观察组患者CD_3^+,CD_4^+,CD_4^+/CD_8^+和自然杀伤(NK)细胞水平显著高于对照组(P <0. 05);观察组不良反应发生率为63. 33%,显著低于对照组的87. 10%(P <0. 05)。结论复方苦参注射液联合多西他赛、奈达铂治疗中晚期宫颈癌临床疗效显著,可改善患者生存质量,减少放、化疗毒副反应,提高机体免疫功能和临床获益率。Objective To investigate the effect of Compound Kushen Injection combined with docetaxel and nedaplatin in the treatment of metaphase or advanced cervical carcinoma and its effect on immune function. Methods Totally 61 patients with metaphase or advanced cervical carcinoma admitted to our hospital from January 2015 to January 2017 were selected and divided into the control group（ n = 31） and the observation group（ n = 30） according to random number table method. On the basis of conventional radiotherapy,the control group was given chemotherapy regimen of Docetaxel Injection and nedaplatin,the observation group was given intravenous drip of Compound Kushen Injection on the basis of the control group. The two groups were treated for 2 courses continuously with 21 d for 1 course.Results After treatment,the total effective rate and the clinical benefit rate of the observation group were 66. 67% and 90. 00%,which were significantly higher than 38. 71% and 61. 29% of the control group（ P〈0. 05）. The overall improvement rate of quality of life in the observation group was 90. 00%,which was significantly higher than 67. 74% in the control group（ P〈0. 05）. After treatment,the levels of CD3^＋,CD4^＋,CD4^＋/CD8^＋ and NK cells in the observation group was significantly higher than those in the control group（ P〈0. 05）. The incidence rate of adverse reactions in the observation group was 63. 33%,which was significantly lower than 87. 10% in the control group（ P〈0. 05）. Conclusion Compound Kushen Injection combined with docetaxel and nedaplatin is effective in the treatment of metaphase or advanced cervical carcinoma,it can improve the quality of life,reduce the toxic side effects of radiotherapy and chemotherapy,and improve the immune function and clinical benefit rate.
