百令胶囊联合辛伐他汀改善糖尿病肾病微炎症状态临床研究

摘　　要：目的观察百令胶囊联合辛伐他汀对糖尿病肾病微炎症状态的改善效果。方法选择医院2013年8月至2017年3月收治的糖尿病肾病患者79例,随机分为治疗组(39例)与对照组(40例)。两组患者均接受糖尿病的基础治疗方案,包括控制饮食和运动疗法,口服降糖药或皮下注射胰岛素,并给予优质低蛋白饮食,口服辛伐他汀片,每日1次,每次20 mg,治疗组患者在此基础上加服百令胶囊,每日3次,每次20 mg。两组患者疗程均为3个月。结果治疗后,治疗组患者24 h尿蛋白定量、C反应蛋白、三酰甘油、白细胞介素6(IL-6)、空腹血糖及总胆固醇水平分别为(0. 83±0. 39) g,(4. 24±1. 32) mg/L,(2. 28±0. 64)μmol/L,(42. 17±10. 27) pg/m L,(7. 29±0. 64)mmol/L,(4. 47±2. 01) mmol/L,较治疗前的(1. 99±0. 72) g,(15. 57±5. 83) mg/L,(3. 23±0. 87)μmol/L,(92. 15±16. 27) pg/m L,(10. 28±3. 17) mmol/L,(6. 54±2. 32) mmol/L,以及对照组治疗后的(1. 59±0. 75) g,(7. 32±1. 99) mg/L,(2. 45±0. 72)μmol/L,(72. 58±10. 37)pg/mL,(9. 02±0. 58) mmol/L,(5. 44±2. 51) mmol/L均显著降低(P <0. 05);两组患者治疗中及治疗后均无全身不良反应发生。结论百令胶囊联合辛伐他汀治疗糖尿病肾病,在减少蛋白尿的同时,能明显降低血清炎性因子及空腹血糖水平,改善微炎症状态。Objective To observe the clinical effect of Bailing Capsules combined with simvastatin on improving microinflammation state of patients with diabetic nephropathy. Methods Totally 79 patients with diabetic nephropathy admitted to our hospital from August 2013 to March 2017 were randomly divided into the treatment group（ 39 cases） and the control group（ 40 cases）. The two groups received basic treatment regimens for diabetes,including diet control and exercise therapy,oral administration of hypoglycemic drugs or subcutaneous insulin injection,and high-quality low-protein diet,oral administration of Simvastatin Tablets,once a day,20 mg per time. On this basis,the treatment group was treated with Bailing Capsules,three times a day,20 mg per time. The two groups were treated for 3 months.Results After treatment,the levels 24 h urinary protein quantification,C-reactive protein（ CRP）,triglyceride,IL-6,fasting blood glucose and total cholesterol in the treatment group were（ 0. 83 ± 0. 39） g,（ 4. 24 ± 1. 32） mg/L,（ 2. 28 ± 0. 64） μmol/L,（ 42. 17 ± 10. 27） pg/m L,（ 7. 29 ± 0. 64） mmol/L,（ 4. 47 ± 2. 01） mmol/L,which were significantly lower than（ 1. 99 ± 0. 72） g,（ 15. 57 ± 5. 83） mg/L,（ 3. 23 ±0. 87） μmol/L,（ 92. 15 ± 16. 27） pg/m L,（ 10. 28 ± 3. 17） mmol/L,（ 6. 54 ± 2. 32） mmol/L before treatment,and also significantly lower than（ 1. 59 ±0. 75） g,（ 7. 32 ±1. 99） mg/L,（ 2. 45 ±0. 72） μmol/L,（ 72. 58 ± 10. 37） pg/m L、（ 9. 02 ± 0. 58） mmol/L,（ 5. 44 ± 2. 51） mmol/L in the control group after treatment（ P〈0. 05）. There were no systemic adverse reactions in the two groups during or after treatment.Conclusion Bailing Capsules combined with simvastatin in the treatment of diabetic nephropathy can significantly reduce the levels of serum inflammatory factors and fasting blood glucose while reducing proteinuria,and improve the micro-inflammation state.
