双镜手术及术后腹腔温热灌注化疗在梗阻性左半结肠癌临床治疗中的应用研究

摘　　要：目的研究双镜手术及术后腹腔温热灌注化疗在梗阻性左半结肠癌临床治疗中的应用效果。方法将2017年9月—2018年2月我院收治的118例梗阻性左半结肠癌患者作为研究对象,依照随机数字表法分为2组各59例。对照组实施开腹手术治疗,观察组行双镜手术及术后腹腔温热灌注化疗治疗。观察2组临床疗效、手术相关指标、住院时间、术后并发症、术后转移率与复发率等。结果观察组手术时间、术中出血量、住院时间、并发症发生率、癌症转移率低于对照组,差异有统计学意义(P<0.05);2组复发率相比,观察组低于对照组,但差异无统计学意义(P>0.05)。结论将双镜手术及术后腹腔温热灌注化疗用于梗阻性左半结肠癌治疗中的效果确切且安全性较高,可有效缓解肠梗阻,降低术后相关并发症与肿瘤转移率。Objective To study the application effect of endoscopic-laparoscopic surgery and postoperativeintraperitoneal hyperthermic chemoperfusion（IHCP）in treatment of obstructive left colon cancer. Methods 118 patients withobstructive left colon cancer treated in our hospital from September 2017 to February 2018 were selected,and they were dividedinto two groups by random number table,with 59 cases in each group. Control group was given open surgery,while observationgroup was treated with endoscopic-laparoscopic surgery and postoperative IHCP. The clinical efficacy,surgical indexes,hospital stays,postoperative complications,postoperative metastasis rate and recurrence rate of two groups were observed.Results The operation time,intraoperative bleeding volume,hospital stays,incidence of complications,cancer metastasisrate in observation group were less than those in control group （P〈0.05） .The cancer recurrence rate of observation group waslower than that of control group, but there was no statistical difference（P〉0.05） .Conclusion Endoscopic-laparoscopic surgeryand postoperative IHCP in treatment of obstructive left colon cancer is effective and safe,it can effectively relieve intestinalobstruction,decrease postoperative complications and cancer metastasis rate.
