调脾护心方对心脏外科术后患者肠屏障功能HIF-1α、NF-κB通路的影响

摘　　要：目的探讨调脾护心方对心脏外科术后患者肠屏障功能缺氧诱导因子-1α(HIF-1α)/核因子-κB(NF-κB)通路的影响。方法采用前瞻性双盲随机对照研究方法,将心脏外科术后60例患者随机分为治疗组及对照组,分别选用调脾护心方中药配方颗粒和中药安慰剂配方颗粒进行治疗,疗程均为7 d。所有患者治疗前、术后1 d、术后3 d、术后7 d抽血留样检查HIF-1α/NF-κB。结果 2组患者术后HIF-1α、NF-κB含量均较术前显著增加(P均<0.05),随着术后的康复,2组患者HIF-1α、NF-κB含量呈逐渐下降趋势(P均<0.05)。术前及术后1 d,2组HIF-1α、NF-κB含量比较差异无统计学意义(P均>0.05),术后3 d、术后7 d,治疗组HIF-1α、NF-κB含量较对照组显著下降(P均<0.05)。结论心脏外科手术可导致机体HIF-1α、NF-κB的表达增高,造成肠屏障功能障碍,调脾护心方可下调该通路的表达,从而促进术后康复,减少术后并发症。Objective It is to explore the effect of Tiaopi Huxin formular on intestinal barrier function of HIF-1α and NF-κB pathway in patients after cardiac surgery. Methods A prospective double-blind randomized controlled trial was conducted. 60 patients after cardiac surgery were randomly divided into treatment group and control group,the treatment group was treated with Tiaopi Huxin formula,while the control group was treated with placebo,the course of treatment was 7 days. Before treatment,1 days after operation,3 days after operation,and 7 days after operation,all patients were examined with blood samples for the detection of HIF-1α and NF-κB. Results The contents of HIF-1α and NF-κB were significantly higher in postoperative patients than that in preoperative patients,the contents of HIF-1α and NF-κB were gradually decreased after cardiac operation in the two groups. There was no significant difference in the levels of HIF-1α and NF-κB in both groups before and 1 days after operation. The contents of HIF-1α and NF-κB in the treatment group were significantly lower than those in the control group after 3 days and 7 days of operation. Conclusion Cardiac surgery can lead to an increase in the expression of HIF-1α and NF-κB which lead to intestinal barrier dysfunction. Tiaopi Huxi formula can reduce the expression of HIF-1α and NF-κB,thereby promoting postoperative rehabilitation and reducing postoperative complications.
