补中益气汤联合钙剂、活性维生素D治疗糖尿病性骨质疏松症疗效观察

摘　　要：目的观察补中益气汤联合钙剂、活性维生素D治疗糖尿病性骨质疏松症的疗效及对骨折发生率的影响。方法将148例糖尿病性骨质疏松症患者随机分为2组,对照组74例给予钙剂和活性维生素D治疗,研究组74例给予补中益气汤联合钙剂、活性维生素D治疗,治疗3个月后评定2组临床疗效,记录并比较治疗前后血糖指标、骨密度,骨代谢生化指标,统计2组治疗后1年内骨折发生率。结果治疗后研究组总有效率显著高于对照组(P<0.05);2组空腹血糖(FPG)、餐后2 h血糖(2h PG)、糖化血红蛋白(Hb A1c)、血清Ⅰ型原胶原N段前肽(PINP)水平均显著降低(P均<0.05),且研究组均显著低于对照组(P均<0.05);2组骨密度、胰岛素样生长因子-1(IGF-1)水平均较治疗前显著升高(P均<0.05),且研究组均显著高于对照组(P均<0.05)。研究组治疗后1年内初次和再次骨折发生率均显著低于对照组(P均<0.05)。结论补中益气汤联合钙剂、活性维生素D治疗糖尿病性骨质疏松症疗效显著,同时能明显降低骨折发生率。Objective It is to observe the curative effect of Buzhong Yiqi Decoction combined with calcium and active vitamin D on diabetic osteoporosis and its effect on the incidence of fracture. Methods A total of 148 patients with diabetic osteoporosis were randomly divided into two groups. 74 patients in the control group were treated with calcium and active vitamin D. 74 patients in the study group were given Buzhong Yiqi Decoction combined with calcium and active vitamin D.After treatment for 3 months,the clinical efficacy of the two groups was evaluated. The blood glucose index,bone mineral density,biochemical markers of bone metabolism were recorded and compared before and after treatment. The incidence of fracture within 1 year after treatment was counted. Results After treatment,the total effective rate of the study group was significantly higher than that of the control group（ P〈0.05）; the levels of fasting blood glucose（ FPG）,postprandial 2 hours blood glucose（ 2 h PG）,glycosylated hemoglobin（ Hb A1 c）,serum type I procollagen N segment peptide（ PINP）were significantly decreased in both groups（ P〈0.05）,and the decrease in the study group was more significant than that in the control group（ P〈0.05）; the bone mineral density and the levels of insulin-like growth factor-1（ IGF-1） levels in the 2 groups were higher than those before treatment（ P〈0.05）,and these indexes in the study group were significantly higher than those in the control group（ P〈0.05）. The incidence of initial and recurrent fractures in the study group was significantly lower than that in the control group within 1 year after treatment（ P〈0.05）. Conclusion Buzhong Yiqi Decoction combined with calcium and active vitamin D is effective in the treatment of diabetic osteoporosis,and can significantly reduce the incidence of fracture.
