摘　　要：目的了解慢病管理模式对出院后痛风患者慢性病管理自我效能的影响,以期为提高痛风患者出院后自我管理能力提供依据。方法选取痛风住院患者120例,随机分为对照组和试验组,试验组在常规随访的基础上实施6个月的慢病管理。应用慢性病管理自我效能量表分别在出院前1日、出院后3个月、出院后6个月对两组患者进行评测。结果试验组患者出院后3个月和出院后6个月慢性病管理自我效能量表得分均高于对照组(P <0. 05);试验组患者出院后3个月和出院后6个月慢性病管理自我效能量表得分均高于出院前1日,出院后6个月慢性病管理自我效能量表得分高于出院后3个月。结论慢病管理能够提高出院后痛风患者慢性病管理自我效能。Objective To explore the effect of management of chronic disease model on the self efficacy of chronic disease management in patients with chronic disease after discharge,in order to provide evidence for improving self-management ability of patients with gout. Methods One hundred and twenty patients with gout were randomly divided into the control group and the experimental group. The experimental group was implemented for 6 months on the basis of followup. The two groups of patients were evaluated by application of chronic disease management self-efficacy scale on 1 day before discharge,3 months after discharge,6 months after discharge. Results Chronic disease management self-efficacy scale scores of patients in the test group discharged after 3 months and discharged after 6 months were higher than those in the control group（P〈 0. 05）; chronic disease management self-efficacy scale scores of patients in the experimental group discharged after 3 months and discharged after 6 months were higher than 1 day before discharge,and chronic disease management self efficacy scale score of patients discharged after 6 months were higher than discharged after 3 months. Conclusion Management of chronic disease model can improve chronic disease management self-efficacy of patients with gout after discharge.
