摘 要:目的:探讨过氧化物酶体增殖物激活受体γ2(PPARγ2)基因Pro12Ala多态性与原发性高血压(EH)的关系。方法:全面检索PubMed、Medline、万方数据库、中国知网(CNKI)、维普资讯,收集关于PPARγ2基因Pro12Ala多态性与EH发生关系的病例-对照研究,采用优势比(OR)和95%可信区间(95%CI)评估关联强度,应用RevMan 5.3软件对纳入的研究进行异质性检验和效应值OR合并,漏斗图评估发表性偏倚。结果:共18篇文章被纳入研究,包括20项病例-对照研究,累计HE患者4 492例和对照组人群5 168例。Meta分析结果显示:总体人群PPARγ2基因Pro12Ala各基因型模型及Ala等位基因均与EH相关,根据区域种群不同进一步进行亚组分析,结果显示PPARγ2基因Pro12Ala各基因型模型及Ala等位基因在亚洲人群中与EH有相关性,而在高加索人群中与EH无关。同时,漏斗图未检测出显著的发表性偏倚,且敏感度分析过程显示目前所得的结果是稳定的。结论:PPARγ2基因Pro12Ala多态性在亚洲人群与EH易感性相关,而在高加索人群中与EH无相关性。Objective:To evaluate the relationship between peroxisome proliferator-activated receptor γ2(PPARγ2)Pro12Ala genetic polymorphisms and the risk of essential hypertension(EH).Method:Relevant literatures were extensively searched in PubMed,Medline,Wanfang Databases,CNKI and VIP for collecting the casecontrol studies investigating the relationship between PPARγ2 Pro12Ala genetic polymorphisms and EH.Odds ratios(OR)with 95% confidence intervals(CI)were applied to assess the strength of association.The RevMan5.3 software was applied for heterogeneity test and combined ORand their 95%CIcalculation.Publication bias was assessed through funnel plot.Result:Eighteen related literatures(Twenty case-control studies)on the relationship between PPARγ2 Pro12Ala genetic polymorphisms and EH were included,involving 4 492 patients(case-groups)and 5 168 controls.Meta-analysis results showed that:all of PPARγ2 gene Pro12Ala genotype models and Ala allele were associated with EH in the general population,and,the results of subgroup analysis according to the different regional population showed that PPARγ2 gene Pro12Ala genotypes and Ala allele were related to EH in Asian populations,but not in the Caucasus populations.At the same time,no significant publication bias was detected in the funnel plot,and the process of sensitivity analysis showed that the results were stable.Conclusion:The Pro12Ala genetic polymorphism of PPARγ2 was associated with the increased risk of EH in Asian populations,but not in the Caucasus populations.
