摘 要:目的探讨结节性痒疹(prurigo nodularis,PN)皮损中IL-31及其受体的表达情况。方法留取PN患者皮损中心、皮损周围及正常皮肤标本,通过实时荧光定量PCR和免疫组化染色方法,分别从转录水平和蛋白水平比较各组之间IL-31、IL-31受体α(IL-31RA)和肿瘤抑制素受体(OSMR)的表达差异。结果 PN皮损处、皮损周围和正常皮肤相比,皮损周围IL-31 mRNA相对表达量略高于皮损处和正常皮肤,三者之间的差异无统计学意义(P=0. 55)。PN皮损周围IL-31RA相对表达量高于皮损处和正常皮肤,差异有统计学意义(P <0. 05),皮损处IL-31RA表达高于正常皮肤,但是差异无统计学意义(P=0. 11)。PN皮损处、皮损周围和正常皮肤OSMR表达量差异无统计学意义(P=0. 23)。免疫组化染色结果显示PN皮损处真皮内部分炎症细胞IL-31呈阳性染色,PN皮损处真皮内神经束IL-31RA呈阳性染色,OSMR广泛表达于表皮、真皮内炎症细胞及皮肤附属器上皮,皮损处真皮神经束也呈阳性染色。结论 PN皮损内炎症细胞可产生IL-31,皮损处真皮神经束表达IL-31受体双链IL-31RA和OSMR,提示IL-31及其受体可能参与PN瘙痒的发生。Objective To investigate the expression of IL-31 and its receptors in the lesions of prurigo nodularis(PN). Methods Lesional and perilesional skin of PN together with normal skin specimens were collected. The mRNA and protein levels of IL-31,IL-31 RA and OSMR were compared by real-time fluorescence quantitative PCR and immunohistochemical staining respectively. Results Compared with the normal skin and PN lesion skin,the relative expression of IL-31 mRNA of perilesional skin increased slightly,but the difference showed no statistically significant(P = 0. 55).Expression of IL-31 RA mRNA in perilesional was higher than the lesion and normal skin(P〈0. 05). The expression of IL-31 RA in lesion was higher than the normal skin,but the difference was no statistically significant(P = 0. 11). There was no significant difference in the OSMR expression level between lesion,perilesional and normal skin(P = 0. 23). Immunohistochemical staining showed that IL-31 was positive in the inflammatory cells in lesions. IL-31 RA was positive in nerve bundles of le-sions. OSMR was widely expressed in epidermis,dermis inflammatory cells and epithelium of skin appendage. OSMR was also positive in nerve bundles of lesions. Conclusion The inflammatory cells in PN lesions can produce IL-31,IL-31 RA and OSMR,which suggests that IL-31 pathway may be involved in the pathogenesis of PN.
