摘　　要：目的探讨急性泛发性发疹性脓疱病(acute generalized exanthematous pustulosis,AGEP)患者外周血血清中IL-1β、IL-4、IL-10、IL-17a水平变化,以便更好地认识AGEP的外周血免疫学病理变化,为AGEP的治疗提供指导。方法采用ELISA法对观察组(37例AGEP患者)、对照1组(40例发疹型药疹)和对照2组(40例健康体检者)的外周血血清进行IL-1β、IL-4、IL-10、IL-17a的测定,对比组间差异。结果观察组与对照1组相比,外周血血清IL-4水平有所降低,差异有统计学意义(P <0. 05),而外周血血清IL-17a水平有显著升高,差异有统计学意义(P <0. 01);观察组与对照2组相比,外周血血清IL-1β、IL-4、IL-10、IL-17a水平均有显著升高,差异均有统计学意义(P <0. 01)。结论 AGEP的外周血免疫学病理变化与发疹型药疹明显不同,以IL-17a升高为主,治疗方案建议尽早使用对Th17细胞有抑制作用的药物。Objective To further the studies of the immunological pathogenesis of AGEP,and to provide suggestion for the treatment of AGEP by detecting the expression of IL-1β,IL-4,IL-10,IL-17 a level in peripheral blood among acute generalized exanthematous pustulosis（AGEP） patients. Methods Three groups were established： 37 patients with AGEP（observation group）,40 patients with exanthematous drug eruption（control group 1）,and 40 healthy people（control group 2）. The concentration of IL-1β,IL-4,IL-10,IL-17 a in peripheral blood from all of the three groups was tested by ELISA.Results Compared with control group 1,the concentration of IL-4 in observation group was decreased（P〉0. 05）,and the concentration of IL-17 a was increased（P〉0. 01）; compared with control group 2,the concentration of IL-1β,IL-4,IL-10,IL-17 a in observation group was increased,the difference was statistically significant（P〉0. 01）. Conclusion The immunological pathological changes in peripheral bloodof AGEP are different with exanthematous drug eruption,which is characterized by significant increasing of IL-17 a. Therefore,Th17 cell inhibitor drug should be prescribed to patients with AGEP as soon as possible.
