摘　　要：目的分析2002-2017年阆中市艾滋病流行特征和变化趋势,为制定防控策略提供科学依据。方法从全国艾滋病综合防治数据信息系统导出报告病例相关历史卡片,用Excel 2010建立数据库,采用SPSS20. 0进行数据分析。结果2002-2017年阆中市累计报告艾滋病病毒(HIV)感染者/艾滋病(AIDS)病人371例(其中HIV感染者282例,AIDS89例),报告死亡116例。HIV/AIDS报告发病率总体呈上升趋势(χ~2趋势=388. 31,P <0. 001)。男女性别比为3. 17∶1,男性女性发病率均呈上升趋势。年龄以20~49岁青壮年为主占60. 37%,50岁及以上新报告占比逐年增加。职业以农民和民工为主,占48. 25%,其次是家务与待业,占20. 22%。异性传播和男男同性传播分别占82. 75%、13. 75%。结论阆中市艾滋病疫情总体呈上升趋势,青壮年、农民及民工居多,50岁以上发病率增长最快,性接触为主要传播途径,应采取多种针对性措施控制艾滋病经性途径传播。Objective To analyze the epidemiological trend and characteristics of HIV/AIDS in Langzhong from 2002 to 2017, so as to provide the scientific evidence for developing an intervention strategy. Methods Basic database of cases was derived from the national AIDS comprehensive data information system, and data were examined and analyzed by software Epi Info. Results From 2002 to 2017, there were 371 HIV/AIDS cases （ consisting of 282 HIV cases and 89 AIDS cases） with 116 deaths in Langzhong, and the incidence of HIV/AIDS was on the rise （χ^2 =388.31, P 〈0.001 ）. The male/female incidence ratio was 3.17：1, and the incidence of male/female increased. The proportion of cases aged from 20 to 49 was 60.37% , and the over - 50 - year - olds cases were increasing annually. The affecting peasant was 48.25% , and household workers or unemployment were 20.22%. The infection transmission rates of heterosexuals and male homosexuals were 82.75% and 13. 75%, respectively. Conclusion The epidemic of HIV/AIDS is on a rapid rising trend in Langzhong, and mainly affecting young adults and peasants. The over - 50 - year - olds cases are increasing, and the heterosexual and male homosexual transmission is the major route. Multiple targeted strategies should be adopted to control the sexual transmission.
