摘 要:目的探讨脂氧合酶基因多态性与DKD合并心血管疾病(CVD)、颈动脉内-中膜厚度(CIMT)的关系。方法选取2014年2月至2018年3月于我院住院的477例DKD患者,根据是否合并CVD分为合并CVD组(CVD,n=372)和无CVD组即对照组(Con,n=105)。根据CIMT分为正常组(CIMT<1.0mm)119例及增厚组(CIMT≥1.0mm)358例。采用TaqMan-PCR检测5-脂氧合酶基因多态性,分析不同基因型与DKD、CVD、CIMT的关系。结果 Con组GG基因型分布频率、G等位基因频率与CVD组比较,差异有统计学意义(χ~2=6.598、16.162,P<0.01)。CIMT正常组GG基因型分布频率、G等位基因频率与增厚组比较,差异有统计学意义(χ~2=15.211、31.419,P<0.01)。Logistic回归分析显示,年龄、高血压病史、LDL-C、C-RP、β2微球蛋白(β2-MG)、CIMT、阻力指数(RI)及左室心肌重量指数(LVMI)为DKD合并CVD事件的危险因素。GG型、TG型年龄、C-RP、β2-MG、CIMT、RI及LVMI与TT型比较,差异有统计学意义(P<0.05或P<0.01)。结论5-脂氧合酶激活蛋白(ALOX5AP)rs17222919TT基因型为DKD合并CVD的易感基因,与CIMT、LVMI增加有关。Objective To investigate the relationship between 5-1ipoxygenase (ALOX5AP) gene polymorphism and cardiovascular disease in diabetic kidney disease (DKD)and carotid artery intima-media thickness (CIMT). Methods Totally 477 patients with DKD were enrolled in this study. DKD patients were divided into two groups according to the combination with cardiovascular disease (CVD group, n= 372) or without CVD (Con group, n= 105). According to CIMT, the subjects were divided into normal carotid artery group (CIMT〈1. 0 ram, n=l19) and carotid artery thickening group (CIMT≥1. 0 mm, n=358). TaqMan technology was used to detect the gene polymorphism of ALOX5AP(rs17222919). Results The frequencies of GG genotype and G allele in Con group were significantly different from those in CVD group (P〈0. 01). The frequencies of GG genotype and G allele in carotid artery normal group were significantly different from those in carotid artery thickening group (P〈0. 01). Logistic regression analysis showed that age, the history of hypertension, LDL-C,C-RP, β2-MG, CIMT, RI and LVMI were risk factors of DKD combined with CVD. The levels of C-RP, β2-MG, CIMT, RI and LVMI in patients With GG and TG genotype were significantly different from TT genotype (P〈0.05 or P〈0.01). Conclusion ALOX5AP (rs17222919) TT genotype is susceptible for DKD with CVD, which is associated with increased CIMT and left ventricular myocardial mass index.
