摘 要:目的探讨T2DM患者血清脂氧素A4(LXA4)与早期糖尿病肾脏疾病(EDN)的相关性。方法选取新诊断40例单纯T2DM患者、38例EDN患者及40名健康体检者(NC组)。ELISA测定空腹血清LXA4水平。结果 NC组、T2DM组、EDN组血清LXA4水平依次降低[(551.48±52.71)vs(502.63±48.57)vs(464.78±42.56)pg/ml,(P<0.05或P<0.01)];血清LXA4水平与血红素加氧酶1(HO-1)呈正相关(r=0.512,P<0.05),与胰岛素抵抗指数(HOMA-IR)、FIns、FPG、HbA_1c、血管内皮生长因子(VEGF)、UACR、病程、转化生长因子β1(TGF-β1)、白细胞介素6(IL-6)、BUN、Scr呈负相关(r=-0.612、-0.512、-0.532、-0.552、-0.528、-0.598、-0.596、-0.603、-0.617、-0.598、-0.550,P<0.05或P<0.01);Logistic回归分析显示,HbA_1c、LXA4是EDN的独立影响因素。结论 EDN患者血清LXA4水平降低,在EDN中发挥重要作用。Objective To explore the association between serum lipoxin A4 (LXA4) and early type 2 diabetic nephropathy (EDN). Methods 40 patients with newly diagnosed type 2 diabetes (T2DM group), 38 patients with early diabetic nephropathy(EDN group) and 40 healthy people with normal glucose tolerance (NC group) were selected. Serum LXA4 was measured by ELISA method. Results The serum LXA4 levels were higher in NC group (551.48±52.71) pg/ml than those in T2DM group (502. 634±48.57) pg/ml and EDN group (464. 784±42.56) pg/ml(P〈0.05 or P〈0.01). Correlation analysis showed that LXA4 levels were positively correlated with heme oxygenase-1 (HO-1) (r= 0. 512, P〈0.05) and negatively correlated with HOMA-IR, Fins, FPG, HbAlc, VEGF, UACR, duration, TGF-t31, 1L-6, BUN and Scr(r=-0. 612, -0. 512, -0. 532, -0. 552, -0. 528, -0. 598, -0. 596, -0. 603, -0. 617, -0. 598 and -0. 550 respectively, P〈0.05 or P〈0. 01), Logistic regression analysis showed that LXA4 and HbA1 c were independent influence factors for EDN. Conclusion Serum LXA4 is lower in EDN patients and may play an important role in the development of EDN.
