摘 要:目的观察二甲双胍对T2DM大鼠肾组织转化生长因子β1(TGF-β1)mRNA和蛋白表达的影响,探讨其对DKD的保护机制及与降糖效应的相关性。方法SD大鼠高脂饮食联合小剂量STZ腹腔注射建立T2DM模型,成模后随机分为T2DM组(n=10)、二甲双胍组[300mg/(kg·d),MET,n=8]和格列本脲组[5mg/(kg·d),GLY,n=8],设置正常对照组(NC,n=9)。干预8周后,检测各组FPG、HbA_1c,UAlb、尿肌酐(Ucr)、尿TGF-β1水平以及肾组织中TGF-β1mRNA和蛋白的表达,电镜观察肾小球基底膜厚度(GBMT)。结果8周末,与NC组比较,T2DM组TGF-β1 mRNA[(7.10±0.20)vs(1.00±0.07)]、TGF-β1光密度值(IOD)[(44.66±5.57)vs(14.08±1.98)]、尿TGF-β1与Ucr比值(UTCR)[(187.54±10.14)vs(34.51±4.67)ng/g]、FPG[(15.60±1.56)vs(4.45±1.07)mmol/L]、HbA_1c[(12.41±0.61)%vs(4.13±0.89)%]、GBMT[(266.58±21.68)vs(106±10.23)nm]、UACR[(136.31±6.84)vs(34.09±2.92)mg/g]升高(P<0.05);与T2DM组比较,MET组TGF-β1 mRNA[(2.60±0.09)vs(7.10±0.20)]、TGF-β1 IOD[(21.85±6.54)vs(44.66±5.57)]、UTCR[(63.74±5.29)vs(187.54±10.14)ng/g]、GBMT[(150.98±17.25)vs(266.58±21.68)nm]、UACR[(98.92±5.40)vs(136.31±6.84)mg/g]降低(P<0.05);与GLY组比较,MET组TGF-β1mRNA[(2.60±0.09)vs(4.69±0.12)]、TGF-β1 IOD[(21.85±6.54)vs(37.43±3.14)]、GBMT[(150.98±17.25)vs(203.67±23.32)nm]、UTCR[(63.74±5.29)vs(86.49±6.61)ng/g]、UACR[(98.92±5.40)vs(110.10±6.76)mg/g]降低(P<0.05),两组FPG和HbA_1c比较,差异无统计学意义。结论二甲双胍降低糖尿病肾组织中TGF-β1水平,发挥肾脏保护作用,可能部分依赖于其降糖之外的作用。Objective To observe the effect of metformin on the expression of TGF-β1 mRNA and protein in renal tissue of type 2 diabetic (T2DM) rats, and to investigate the protective effect of metformin on diabetic kidney disease (DKD) and whether this effect is related to the hypoglycemic effect of metformin Methods SD rat fed with high-fat diet was intraperitoneally injected a small dose of STZ to establish T2DM model. The model rats were randomly divided into T2DM group (n= 10), metformin group [300 mg/(kg . d), MET, n=8] and glibenclamidegroup [5 mg/(kg . d), GLY, n=8]. 9 normal rats were set as control group (NC). After intervention for 8 weeks, the level of FPG, HbAlc, urinary albumin (UAlb), Urine creatinine(Ucr), urinary TGF-β1, TGF-β1 mRNA and protein expression in renal tissue were measured. Glomerular basement membrane thickness (GBMT) was observed by electron microscope. Results At the end of 8th week, the levels of TGF-β1 mRNA[(1.00± 0.07) vs (7.10± 0.20), TGF-β1 IOD[(44.66±5.57) vs (14. 08±1.98)], urinary TGF-β1/Uer (UTCR) [(187.54〉10.14) vs (34.51±4.67) ng/g], FPG [(15.6±1.56) vs (4.45±1.07) mmol/L], HbA1c [(12.41± 0.61)% vs (4. 13±0.89)%], GBMT[(266.58±21.68) vs (106±10.23)nm] and urinary albumin/Ucr (UACR) [(136.31 ± 6.84) vs (34.09 ± 2.92) mg/g] increased significantly in T2DM group compared with NC group (P〈0. 05). Compared with T2DM group, TGF-β1 mRNA[(2. 60±0. 09)1 vs (7.10± 0. 20), TGF-β1 IOD [(21. 85±6. 54) vs (44.66±5.57)], UTCR [(63.74±5.29) vs (187.54±10.14) ng/g],GBMT[(150. 98±17. 25) vs (266.58±21.68) nm] and UACR [(98.92±5.40) vs (136.31± 6.84) mg/g] in MET group decreased significantly (P〈0. 05).
