摘 要:目的:探讨痛泻要方对腹泻型肠易激综合征(IBS-D)肝郁脾虚证大鼠模型的干预作用及其机制。方法:SD大鼠随机分为正常组、模型组、中药组,每组10只。采用母子分离联合慢性束缚应激和番泻叶灌胃法建立IBS-D肝郁脾虚证病证结合大鼠模型,采用自制测压计检测各组大鼠的痛觉阈值,观察造模结束后的大便积分,采用旷场实验、糖水偏好实验及悬尾实验观察大鼠行为学特征,采用流式细胞仪分析血液中T淋巴细胞亚群,间苯三酚法检测血清D-木糖含量,ELISA法检测血清免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、5-羟色胺(5-HT)、脑源性神经营养因子(BDNF)水平,甲苯胺蓝改良染色法检测近端结肠组织中的肥大细胞(MC),酶免疫组化法检测近端结肠和末端回肠组织中的嗜铬细胞(EC)。结果:与正常组相比,模型组大鼠痛觉阈值明显降低,大便积分增加,旷场穿格数、站立数明显减少,糖水偏好率降低,悬尾不动时间延长(P<0.05);血IgA水平升高,T淋巴细胞CD3+CD4+亚群比例升高,CD3+CD8+亚群比例降低,CD4+/CD8+比值升高(P<0.05);血清D-木糖含量降低、5-HT水平升高(P<0.05);近端结肠和末端回肠EC数量增加,近端结肠MC数量增加(P<0.05)。与模型组比较,中药组大鼠的痛觉阈值升高,旷场穿格数、站立数升高,糖水偏好率升高,悬尾不动时间缩短(P<0.05);血清5-HT水平降低,末端回肠EC数量减少,近端结肠MC数量减少(P<0.05)。结论:痛泻要方可降低IBS-D肝郁脾虚证模型大鼠的内脏敏感性,改善其抑郁焦虑样行为,降低血清5-HT水平,减少肠组织中EC和MC的数量。Objective: To explore the effect of Tongxieyao Formula (TF) in treating irritable bowel syndrome with predominant diarrhea (IBS-D) rats with syndrome of stagnation of liver qi and spleen deficiency. Methods: Thirty male SD rats were randomly divided into normal control group, model group and TF group, each with ten. Pain threshold was determined by abdominal withdrawal reflex (AWR). The fecal scores were calculated after the modeling. The behavioral characteristics of rats were evaluated by open field test, sucrose preference test and tail suspension test. The T-lymphocyte subsets were analyzed by flow cytometer. The levels of serum D-xylose, IgA, IgG, 5-HT, brain derived nerve growth factor (BDNF) were detected by phloroglucinol colorimetry and ELISA. Enterochromaffin cell (EC) and mast cell counts in intestinal tissues were measured by immunoenzymatic histocbemical technology. Results: Compared with the normal group, pain threshold of rats in the model group was obviously decreased, fecal scores were increased, the total cross number and standing number in the open field test were significantly decreased, the rate of sucrose preference was decreased, non-movement time in the tail suspension test was increased, these differences were statistically significant (P〈0.05). The proportion of CD3^+CD4^+ and the ration of CD3^+CD4^+ increased, while the proportion of CD3^+CD4^+decreased, these differences were statistically significant (P〈0.05). The levels of serum IgA, 5-HT were increased, while D-xylose levels were decreased, these differences were statistically significant (P〈O.05). The EC and MC counts increased in the proximal colon and ileum, these differences were statistically significant (1~〈0.05). Compared with the model group, pain threshold of rats in the TF group was obviously increased, the total cross number and standing number in the open field test were significantly increased, the rate of sucrose preference was increased, non-movement time
