摘  要:目的探讨产妇子宫平滑肌组织中谷胱甘肽过氧化酶1(glutathione peroxidase-1,GPX1)及核因子-κB(NF-κB)表达水平与宫缩乏力性产后出血的相关性。方法选取宫缩乏力性产后出血产妇40例为病例组,另选同期无产后出血的产妇38例为对照组。以等长张力测定法检测各组产妇离体子宫平滑肌的收缩功能及缩宫素诱导的收缩潜能;以羊水压积测定法测定产后24h出血量;采用实时荧光定量(qRT-PCR)技术检测产妇子宫平滑肌组织中GPX1、NF-κB mRNA的相对表达量;采用蛋白免疫印迹(Western blot)技术检测产妇子宫平滑肌组织中GPX1、NF-κB蛋白的表达水平,并对TLR4、COX-2的表达水平及产后出血量与子宫平滑肌的收缩功能进行相关性分析。结果病例组产妇子宫平滑肌收缩频率、收缩活动力均显著低于对照组(P <0. 05);病例组产后24h出血量显著高于对照组(P <0. 05);病例组产妇子宫平滑肌组织中GPX1mRNA及蛋白的表达均显著低于对照组(P <0. 05);病例组产妇子宫平滑肌组织中NF-κB mRNA及蛋白的表达均显著高于对照组(P <0. 05);子宫平滑肌收缩活动力与组织中GPX1 mRNA及蛋白的表达均呈显著正相关(|r|均>0. 4,P均<0. 05),与组织中NF-κB mRNA及蛋白的表达均呈显著负相关(|r|均>0. 4,P均<0. 05),产妇产后24h出血量与子宫平滑肌收缩活动力呈显著负相关(r=-0. 528,P <0. 05)。结论病例组产妇子宫平滑肌组织中GPX1表达水平的下调及NF-κB表达水平的上调可能共同参与调控宫缩乏力性产后出血的发展。Objective To explore the relationship between smooth muscle tissue glutathione peroxidase-1(GPX1) and nuclear factor-κB (NF-κB) expression levels in uterus and uterine atony postpartum hemorrhage. Methods Forty cases of parturient women with uterine atony postpartum hemorrhage were selected as the case group, and 38 cases of parturient women without postpartum hemorrhage as the control group. Contractile function of isolated uterine smooth muscle and oxytocin induced contractile potential of parturient women in each group was detected by using isometric tension measurement. 24h postpartum hemorrhage volume was tested by using amniotic fluid volume determination. Relative expression of GPX1, NF-κB and mRNA in smooth muscle tissue of parturient women was detected by using real-time fluorescence quantitative (qRT-PCR) technique, and GPX1, NF-κB proteins expression levels in maternal smooth muscle tissue, and the correlation was analyzed between TLR4 and COX-2 expression levels and postpartum hemorrhage amount and contractile function of uterine smooth muscle. Results Contraction frequency and contractile activity of the uterine myometrium in the case group were significantly lower than those in the control group ( P 〈0.05). 24h postpartum hemorrhage volume in the case group was significantly higher than that in the control group ( P 〈0.05). GPX1 mRNA and protein expression in smooth muscle tissue of parturient women was significantly lower than that of the control group ( P 〈0.05). NF-κB mRNA and protein expression in the uterine smooth muscle tissue of the case group were significantly higher than those of the control group ( P 〈0.05). Contractile activity of uterine smooth muscle was positively correlated with GPX1, mRNA and protein expression in tissues (| r |〉0.4, P 〈0.05), and negatively correlated with NF-κB mRNA and protein expression in tissues ( |r| 〈0.4, P 〈0.05), and was negatively related to 24h postpartum hemorrhage ( r =-0.528, P �
