靶点射频热凝术联合内热式针灸治疗混合型腰腿痛的临床观察

摘　　要：目的观察靶点射频热凝术联合内热式针灸治疗混合型腰腿痛的的临床疗效。方法将符合标准的80例患者随机分为对照组与观察组,分别进行靶点射频热凝术治疗、靶点射频热凝术联合内热式针灸治疗,观察两组治疗前后的改良日本骨科协会下腰痛评定量表(JOA)评分及临床优良率、Oswestry功能障碍指数(ODI)。结果两组术后2周、3个月、6个月JOA评分及ODI与术前相比均明显升高(P<0.05),且术后3个月及6个月的JOA评分及ODI与术后2周比较差异有统计学意义(P<0.05);观察组术后2周、3个月、6个月的JOA评分、临床优良率及ODI与对照组比较,差异均有统计学意义(P<0.05)。结论靶点射频热凝术联合内热式针灸治疗混合型腰腿痛,是兼顾椎管内外病变的一种安全有效的治疗方法,但其远期疗效尚有待于进一步研究。Objective： To observe the clinical effect of target radiofi＇equency thermocoagulation combined with internal thermal acupuncture on mixed pain in waist and lower extremities. Methods： 80 patients were randomly divided into control group and observation group,treated with target radiofrequency thermocoagulation and target- ed radiofrequency thermocoagulation combined with endothermic acupuncture,respectively. The JOA scores and clinical excellent rates and and the Oswestry disability- index（ODI） were collected to evaluate the the curative ef- fect. Results ： The JOA scores and ODI of the two groups at 2 weeks,3 months and 6 months after treatnient were significantly higher than those before treatntent（P〈0.05）. The data of 3 months and 6 months had significant dif- ference compared with that of 2 weeks （P〈0.05）. There was significant difference between two groups in JOA score, clinical excellent rate and ODI （P〈 0.05 ）. Conclusion： Target radiofi＇equency thermocoagulation combined with internal thermal acupuncture on mixed pain in waist and lower extremities is a safe and effective way that solves both internal and external spinal lesion which deserves attention in clinical theraphy,but its long-term therapeutic effect remains to be further studied.
