孕中、晚期血清同型半胱氨酸水平及其与妊娠高血压和妊娠糖尿病的关系

摘　　要：目的探讨育龄期女性孕中、晚期血清同型半胱氨酸(Hcy)水平的变化及其与妊娠高血压、妊娠糖尿病的关系。方法选取2017年3-12月在该院产检并住院分娩的孕产妇1 035例,分析孕中期、孕晚期Hcy水平变化情况。比较健康组、妊娠糖尿病组、妊娠高血压组及妊娠糖尿病合并妊娠高血压组孕妇在孕中、晚期Hcy水平。结果健康组孕中期Hcy水平为(3.91±1.69)μmol/L[95%参考值范围:(3.80~4.03)μmol/L],孕晚期Hcy水平为(4.14±2.11)μmol/L[95%参考值范围:(4.00~4.28)μmol/L],健康组孕晚期Hcy水平高于孕中期(P<0.05)。孕中期各组间Hcy水平差异均无统计学意义(P>0.05)。孕晚期妊娠高血压组、妊娠糖尿病合并高血压组Hcy水平均高于健康组(P<0.05)。孕晚期妊娠高血压组与妊娠糖尿病合并高血压组孕妇Hcy水平均高于妊娠糖尿病组(P<0.05)。结论健康孕妇Hcy水平在孕中、晚期呈升高趋势。孕晚期Hcy水平与妊娠高血压、妊娠糖尿病合并高血压可能有正相关性。Objective To investigate the change of serum homocysteine（Hcy）levels in middle and late pregnant child bearing women and its relationship with pregnancy-induced hypertension（PIH）and gestational diabetes mellitus（GDM）.Methods A total of 1 035 pregnant and lying-in women undergoing prenatal examination and hospitalization delivery in this hospital from March 2017 to December 2017 were selected.The change situation of Hcy level at the middle and late stages was analyzed.The levels of Hcy at the middle and late pregnant stages were compared among the health group,GDM group,PIH group and GDM complicating PIH group.Results The mean Hcy level at the middle pregnant stage in the health group was（3.91±1.69）μmol/L [95%reference rage：（3.80-4.03）μmol/L],which at the late pregnant stage was（4.14±2.11）μmol/L [95%reference rage：（4.00-4.28）μmol/L].The Hcy level at the late pregnant stage in the health group was higher than that at the middle pregnant stage（P〈0.05）.The Hcy level at the middle pregnant stage had no statistically significant difference among different groups（P〈0.05）.The Hcy level at the late pregnant stage in the PIH group and GDM complicating PIH group was higher than that in the health group（P〈0.05）.The Hcy level at the late pregnant stage in the PIH group and GDM complicating PIH group was higher than that in the GDM group（P〈0.05）.Conclusion The Hcy level in the middle and late pregnant stages shows the increasing trend.The Hcy level at the late pregnant stage may has a positive correlation with PIH and GDM complicating PIH.
