不同剂量糖皮质激素辅助治疗小儿难治性肺炎支原体肺炎合并肺外并发症疗效比较分析

摘　　要：目的比较分析不同剂量糖皮质激素辅助治疗小儿难治性肺炎支原体肺炎合并肺外并发症疗效。方法选取本院儿科2016年7月～2018年2月收治的94例难治性肺炎支原体肺炎合并肺外并发症的小儿患者作为观察对象,依据1:1比例分为对照组(47例,给予常规剂量糖皮质激素治疗)和试验组(47例,给予小剂量糖皮质激素治疗),对比两组临床治疗效果的差异。结果将两组患儿的治疗总有效率展开比较,对照组与试验组组间比较差异无统计学意义;将两组患儿的咳嗽消失时间、喘息消失时间、住院时间展开比较,试验组与对照组比较差异无统计学意义;将两组患儿的不良反应发生率展开比较,试验组低于对照组,组间差异具有统计学意义(P<0.05)。结论小剂量与常规剂量糖皮质激素对小儿难治性肺炎支原体并发肺外并发症均可取得良好的治疗效果,但小剂量更具安全性,无明显不良反应,值得优先选择。Objective To compare the efficacy of different doses of glucocorticoids in the treatment of children with pneumonia complicated with extrapulmonary complications. Methods A total of 94 pediatric patients with refractory mycoplasma pneumonia complicated with extrapulmo- nary complications who were admitted to the pediatrics department of our hospital from July 2016 to February 2018 were selected as objects of obser- vation. According to the 1：1 ratio, they were divided into the control group （47 cases were treated with conventional dose of glucocorticoid） and the experimental group （47 cases were treated with low-dose glucocorticoid）, the difference in clinical efficacy between the two groups was compared. Results The total effective rate of treatment was compared between the two groups of patients. The difference between the control group and the ex- perimental group was not statistically significant; the cough disappearance time, wheezing disappearance time, and length of hospital stay were com- pared between the two groups of children. There was no statistical difference between the experimental group and the control group; the incidence of adverse reactions in the two groups of children was compared, and the experimental group was lower than the control group, with significant differ- ences between the groups （P 〈0.05）. Conclusion Small doses and conventional doses of glucocorticoids can achieve good therapeutic effects on pe- diatric refractory pneumonia complicated with extrapulmonary complications. However, small doses are safer and have no obvious side effects.
