摘 要:目的探讨荧光定量PCR技术检测结核DNA在不同类型样本中的阳性率。方法收集2016年1月~2018年3月在我院进行结核DNA检测的9020例患者,运用荧光定量PCR技术检测不同类型的样本。结果 9020例患者中,检测结果阳性1220例,总阳性率13.53%;痰液检测5688例,检测结果阳性741例,总阳性率13.03%;肺泡灌洗液检测1799例,检测结果阳性363例,总阳性率20.20%;脑脊液检测704例,检测结果阳性40例,总阳性率5.68%;胸水检测559例,检测结果阳性35例,总阳性率6.26%;腹水检测62例,检测结果阳性6例,总阳性率9.68%;尿液检测193例,检测结果阳性35例,总阳性率18.13%;心包积液检测15例,检测结果阳性0例,总阳性率0。结论荧光定量PCR技术检测结核DNA具有特异、灵敏、快速、准确等特点,但临床医生应根据症状体征等选择合适的临床标本进行结核DNA的检测,将进一步提高阳性率,明确诊断。Objective To investigate the positive rate of tuberculosis DNA in different types of samples by fluorescence quantitative PCR.Methods From January 2016 to March 2018,9020 cases of tuberculosis DNA were detected in our hospital,and different types of samples were detected by fluorescence quantitative PCR technique.Results Among 9020 patients,the test results were positive in 1220 cases,the total positive rate was 13.53%;the sputum test was 5688 cases,the test result was positive 741 cases,the total positive rate was 13.03%;the alveolar lavage fluid test was 1799 cases,the test result was positive 363 cases,the total positive rate was 20.20%; the cerebrospinal fluid was detected in 704 cases,the test result was positive in 40 cases,the total positive rate was 5.68%;the pleural effusion test was 559 cases,the test result was positive in 35 cases,the total positive rate was 6.26%;the ascites test was 62 cases,and the test result was positive in 6 cases.The total positive rate was 9.68%;the urine was detected in 193 cases, the test result was positive in 35 cases,the total positive rate was 18.13%;the pericardial effusion was detected in 15 cases,the test result was positive in 0 case,and the total positive rate was 0.Conclusion Fluorescence quantitative PCR is a specific,sensitive,rapid and accurate method for detecting tuberculosis DNA.However,clinicians should select appropriate clinical specimens for the detection of tuberculosis DNA according to symptoms and signs,which will further improve the positive rate and confirm the diagnosis.
