摘　　要：目的探讨常规检验项目回归模型对妊娠高血压综合症的早期诊断价值。方法回顾性分析2017年1月至2017年4月南京医科大学第一附属医院的妊高症孕妇55例,记录孕中期的肌酐(CRE)、尿素(BUN)、尿酸(UA)、红细胞(RBC)计数、血红蛋白(Hb)、血糖(GLU)、β_2氐微球蛋白(β_2-MG)、胱抑素C(CYSC)和视黄醇结合蛋白(RBP)结果,50例非妊高症孕妇作为对照组,进行统计分析。结果 CRE,BUN,UA,GLU,CYSC和RBP均有不同程度的升高,差异有统计学意义(P<0.05);受试者工作特征(ROC)曲线的曲线下面积(AUC)最高的是CYSC为0.802,其次为RBC的0.787。用前进法筛选的CYSC,BUN以及CRE构建的模型,对妊高症的诊断价值达到了0. 935,特异性为77.8%,灵敏度为98.2%,约登指数为0.865。结论由CYSC,BUN以及CRE构建的回归模型对妊高症的早期诊断能起到重要的作用。Objective To explore the early diagnosis value on a regression model for pregnancy induced hypertension. Methods A total of 55 pregnant women with pregnancy induced hypertension were enrolled from January 2017 to April 2017 in the First Affiliated Hospital of Nanjing Medical University and analyzed. Values of creatinine （ CRE）, urea （ BUN ）, uric acid （ UA ）, red blood cell （ RBC ）, hemoglobin （ Hb ）, glucose （ GLU ）, β2 microglobulin （ β2- MG ）, cystatin C （ CYSC ） and retinol binding protein （ RBP ） were recorded, while 45 normal pregnant women were selected as the control group. Results The values of CRE, BUN, UA, GLU, CYSC and RBP in the observation group were all significant higher than those in control group（P 〈 0.05 ）. Using the areas under receiver operating characteristic curve （ ROC）, the AUC values were calculated for pregnancy induced hypertension, and the highest was CYSC which was 0. 802, followed by 0. 787 of RBC. The regression model established through method withCYSC, BUN and CRE achieved the AUC of 0. 935, with the specificity of 0. 778, the sensitivity of 0. 982, and the Youden＇ s index reached 0. 865. Conclusion Theregression model established with CYSC, BUN and CRE plays an important role in early diagnosis of pregnancy induced hypertension.
