摘  要:目的探讨血清甘胆酸在乙肝肝癌肝功能损伤中的评价价值。方法选自我院于2015年6月至2017年6月期间收治的乙肝肝癌肝功能损伤患者49例作为观察组;选自我院于2015年6月至2017年6月期间收治的慢性乙肝患者58例作为慢性乙肝组;另选自我院于2015年6月至2017年6月健康体检者50例作为正常组。所有研究对象均于清晨抽取外周静脉血,分离血清标本,采用均相酶免疫法测定甘胆酸含量,采用丙酮酸氧化酶法测定AST和ALT含量,采用化学发光免疫法测定AFP含量。比较各组血清甘胆酸和AFP含量、AST和ALT含量变化,乙肝肝癌肝功能损伤患者甘胆酸诊断效能,及甘胆酸与AST和ALT相关性。结果观察组血清甘胆酸和AFP含量高于慢性乙肝组和正常组(甘胆酸:t=32.8845、43.1258, AFP:t=28.3268、26.3662,P <0.05);慢性乙肝组血清甘胆酸含量高于正常组(t=23.6726,P <0.05);而慢性乙肝组与正常组血清AFP含量比较无统计学差异(t=1.0317,P>0.05)。观察组血清AST和ALT含量高于慢性乙肝组和正常组(慢性乙肝组:t=24.1187、17.4175,正常组:t=44.8428、36.6440,P<0.05);慢性乙肝组血清AST和ALT含量高于正常组(t=27.5922、21.1881,P<0. 05)。观察组甘胆酸与AST和ALT呈线性正相关,而慢性乙肝组和正常组无明显相关性。结论乙肝肝癌肝功能损伤患者血清甘胆酸水平上升,可作为一种评价指标,具有重要研究价值,值得推广应用。Objective To evaluate the clinical value of serum bile acid in liver function impairment of hepatitis B induced liver cancer. Methods 49 cases of hepatitis B liver function injury patients were selected from our hospital from June 2015to June 2017 as the observation group; 58 cases of chronic hepatitis B patients were selected from our hospital from June 2015 to June 2017 as the chronic hepatitis B group ;50 cases of healthy personsin our hospital from June 2015 to June 2017 were selected as the normal group. The peripheral blood samples were collected at early morning and serum samples were isolated. The content of Gan acid and AFP was determined by homogeneous enzyme immunoassay, and the content of AST and ALT was measured by pyruvate oxidase method, and the content of AFP was determined by chemiluminescent immunoassay. Results For the observation group, the serum glycocholic acid and AFP were higher than that in the chronic hepatitis B group and normal group (CG : t = 32. 8845, 43. 1258, AFP : t =28. 3268, 26. 3662, P 〈 0.05) ; For the chronic hepatitis B group, the serum cholic acid content was higher than the normal group ( t =23. 6726, P 〈 0.05 ); Between the chronic hepatitis B group and normal group, the serum AFP content showed no significant difference (t = 1. 0317. P 〉0.05). The serum AST and ALT levels in the observation group were higher than those in the chronic hepatitis B group and the normal group (chronic hepatitis B group: t = 24.1187, 17. 4175, normal group: t = 44. 8428, 36. 6440 ,P 〈 0.05). The serum AST and ALT levels in thechronic hepatitis B group were higher than those in the normal group ( t = 27. 5922, 21. 1881, P 〈 0.05 ). There was a linear positive correlation between cholic acid and AST and ALT in the observation group, but there was no significant correlation between the chronic hepatitis B group and the normal group.
