摘 要:目的评价三维超声心动图自动定量节段性室壁运动异常(SWMA)患者左心室容积以及射血分数的准确性及可行性。方法对40例窦性心律的节段性室壁运动异常患者,分别采用三维自动方法 (3DEA)、三维自动局部微调法(3DEA-R)和三维手动法(3DEM)测量左心室的舒张末容积(EDV)、收缩末容积(ESV)和射血分数(EF),并记录三种方法的分析时间。结果 3DEA-R比3DEA与3DEM的相关性更好,Bland-Altman一致性检验更高;3DEA和3DEA-R平均耗时较3DEM短。3DEA和3DEA-R测值在观察者内及观察者间的重复性好。结论 3DEA-R分析方法可以快捷、准确定量SWMA患者左心室容积和射血分数,可在临床中推广应用。Objective To evaluate the feasibility and accuracy of 3-dimensional algorithm automatically evaluating left ventricular volume and ejection fraction in patients with segmental wall motion abnormalities(SWMA). Methods Left ventricular end-diastolic volume (EDV), end-systolic volume (ESV) and ejection fraction (EF) of 40 patients with segmental wall motion abnormalities in sinus rhythm were measured by three methods, including 3DE automated algorithm (3DEA), 3DE automated algorithm with regional editing (3DEA-R) and 3DE manual algorithm (3DEM). Examination duration among the three methods was recorded. Results Correlation between 3DEA-R and 3DEM was better than between 3DEA and 3DEM, and Bland-Altman test of consistency was higher for 3DEA-R than 3DEA. Average analysis time by 3DEA and 3DEA-R was shorter than by 3DEM. The measurement has good inter and intra observer repeatalility. Conclusions For the patients with SWMA, 3DEA-R can be used to quantify the left ventricular volume and EF accurately and efficiently. It can be applied in clinical practice.
