摘  要:目的采用斑点追踪显像技术评价肾综合征出血热(HFRS)对左室心肌应变的影响。方法纳入15例HFRS患者在入院时和经治疗2周后行心脏超声检查。同时纳入15例健康者作为对照组在基础状态及2周后行心脏超声检查。分析HFRS左室各节段环向应变、左室整体环向应变、左室各节段纵向应变和左室整体纵向应变的变化。结果 HFRS患者经治疗后LVEF较治疗前有所提高,但无统计学差异。HFRS患者治疗前后LVEF与健康者LVEF相比较无统计学差异。HFRS患者入院时左室基底段环向应变、中间段环向应变、心尖段环向应变、整体环向应变、左室各节段纵向应变和左室整体纵向应变较健康者减弱(P<0.05)。治疗2周后左室各节段环向应变、左室整体环向应变、左室各节段纵向应变和左室整体纵向应变较治疗前有所改善(P<0.05)。结论 STI能够敏感地评价HFRS对患者左室收缩功能的损害,为指导临床及时采取保护心肌治疗具有重要意义。Objective Hemorrhagic fever with renal syndrome (HFRS) may cause various forms of damage in patient's heart. The damage of left ventricular strain in HFRS has not been revealed yet. In this study, we investigate the circumferential strain and longitudinal strain in patients with HFRS by means of speckle tracking imaging. Methods Fifteen HFRS patients and fifteen health adults were enrolled in our study. The echocardiography was performed at admission and two weeks after treatment in HFRS patients. The echocardiography was also performed twice in health adults. The circumferential strain of basal segments, middle segments, apical segments, global circumferential strain, the longitudinal strain of anterior wall, lateral wall, posterior wall, inferior wall, septum and global longitudinal strain in HRFS patients before therapy and after therapy were obtained and compared with those in heahh aduhs. ResuLts The LVEF in HFRS patients at admission was lower than that after therapy. The LVEF in HFRS patients showed no significant difference with that in health adults. The circumferential strain of basal segments, middle segments, apical segments, global circumferential strain, the longitudinal strain of anterior wall, lateral wall, posterior wall, inferior wall, septum and global longitudinal strain in HRFS patients were decreased compared with health adults (P〈0.05). After 2 weeks treatment, the circumferential strain and longitudinal strain were significantly improved in HFRS patients and showed no significant difference with those in health. Conclusions HFRS may cause damage of circumferential strain and longitudinal strain of left ventricle. Speckle tracking imaging can evaluate the circumferential strain and longitudinal strain sensitively and may provide a helpful method for detecting heart damage in HFRS.
