无保护会阴分娩法是初产妇分娩方式的首选吗?

摘　　要：无保护会阴分娩技术是中国妇幼保健协会在2010年开展的"促进自然分娩的中国行动"中推荐使用美国妇产科协会推广的无创助产技术,自应用以来在临床产科领域可谓举足轻重。但是,所谓"无保护会阴"是否任由会阴裂伤,如果任由其裂伤是否符合道德伦理,答案显然不是,更不能。本文通过对无保护会阴分娩法的优势以及不足进行论述,明确无保护会阴的合理性,结果认为其不是初产妇分娩方式的首选,并指出其存在不足之处,且提出改进办法,为提高分娩结局提供理论依据。Unprotected perineum delivery technology is a noninvasive midwifery technology that was first advocated by the American Association of Obstetrics and Gynecology. It was recommended by the China Maternal and Child Health Care Association in ＂2010 China to Promote Natural Birth Action＂. The application is very important in the field of clinical obstetrics. However, does the so-called ＂unprotected perineum＂ allow the perineum to break during childbirth？ Obviously, it does not. It is against morality. This article discussed the advantages and disadvantages of the unprotected perineum childbirth and made clear the rationality of the unprotected perineum, but hold that the unprotected perineum childbirth is not the first choice for primipara childbirth. Finally, the improvement measures were put forward to provide theoretical reference for making better birth outcomes.
