NTrap拦截下输尿管硬镜碎石术与输尿管软镜碎石术治疗输尿管超上段结石的疗效比较

摘　　要：目的比较阻石网篮(NTrap)拦截下输尿管硬镜钬激光碎石术与输尿管软镜钬激光碎石术治疗输尿管超上段结石的临床疗效及安全性。方法回顾性分析我院2013年1月～2018年1月收治的74例成功施术的单侧输尿管超上段结石患者的临床资料,施术方式仅为阻石网篮拦截下输尿管硬镜钬激光碎石术(36例)或输尿管软镜钬激光碎石术(38例)。在确定两组资料在一般资料上不存在统计学上的差异后,比较分析两组的手术时间、一期碎石成功率、术后住院天数、严重并发症发生率、首次输尿管镜碎石术后的无石率(SFR)。结果两组手术时间、一期碎石成功率、术后住院天数、首次输尿管镜碎石术后的SFR比较,差异均无统计学意义(P>0.05),严重并发症在本次入组数据内均未发生。结论与输尿管软镜钬激光碎石术相比,NTrap拦截下输尿管硬镜钬激光碎石术处理输尿管超上段结石的疗效是类似的,是可选的替代方案。Objective To compare the clinical efficacy and safety of rigid ureteroscopic holmium laser lithotripsy with NTrap stone-resistance basket and flexible ureteroscopic holmium laser lithotripsy in the treatment of hyper-upper ureteral calculi. Methods The clinical data of 74 patients with unilateral hyper-upper ureteral calculi treated during Jan.2013 and Jan.2018 were retrospectively analyzed,including 36 patients treated with rigid ureteroscopic holmium laser lithotripsy with NTrap and 38 cases treated with flexible ureteroscopic holmium laser lithotripsy.The operation time,one-stage stone-clearance rate,postoperative hospitalization,incidence of serious complications,and stone-free rate （SFR） were compared between the two groups. Results There were no significant differences in the operation time,one-stage stone clearance rate,postoperative hospitalization,and SFR between the two groups （all P 〉0.05）.No severe complications occurred in either groups. Conclusion Rigid ureteroscopic holmium laser lithotripsy with NTrap has comparable efficacy and safety as flexible ureteroscopic holmium laser lithotripsy.It is an alternative treatment for hyper-upper ureteral calculi.
