继发性甲状旁腺功能亢进行甲状旁腺全切加前臂移植术60例临床分析

摘　　要：目的总结60例尿毒症继发性甲状旁腺功能亢进患者行甲状旁腺全切加前臂移植术的临床经验。方法回顾性分析2010年1月～2017年12月我院泌尿外科行甲状旁腺全切加前臂移植术者60例的病史特点、定位诊断、围手术期处理及疗效。结果 60例患者为长期血液透析者(平均透析7.9年),55例有皮肤瘙痒,30例有骨痛,6例身高缩短(平均缩短9cm,最多缩短15cm),14例有转移性钙化。60例患者均行甲状旁腺全切加前臂移植术,术后症状明显改善。结论对具备手术指征的继发性甲状旁腺功能亢进,甲状旁腺全切加前臂移植术是一种安全、有效的治疗方法。Objective To summarize the clinical experience of total parathyroidectomy and forearm transplantation in 60 cases of uremic patients with secondary hyperparathyroidism. Methods The medical history,localization diagnosis,perioperative management as well as curative effects of these cases treated during Jan.2010 and Dec.2017 were retrospectively analyzed. Results All of the 60 cases needed long-term hemodialysis （average 7.9 years）,among whom 55 had cutaneous pruritus,30 had bone pain,6 had shortened height （average cut-down 9cm,maximum 15cm）,and 14 had metastatic calcification.After the operation,all patients recovered well. Conclusion Total parathyroidectomy plus forearm transplantation is a safe and effective therapy for uremic patients who have secondary hyperparathyroidism.
