帕金森病合并睡眠障碍临床特征与血浆肿瘤坏死因子-α水平的相关性

摘　　要：目的:探讨帕金森病合并睡眠障碍患者的临床特征及其与肿瘤坏死因子-α(TNF-α)水平的相关性。方法 :选取2014年1月至2015年12月江苏省淮安市淮安医院神经内科收治的99例帕金森病患者,根据是否合并睡眠障碍,分为睡眠障碍组(n=59)和睡眠正常组(n=40)。应用帕金森病Hoehn-Yahr分级(H-Y分级)评估对比两组患者病情严重程度,酶联免疫吸附试验(ELISA)法测定血浆TNF-α水平。比较两组日常生活能力(ADL)评分。采用Pearson线性相关分析法分析H-Y分级、睡眠质量PDSS-2评分及ADL评分与血浆TNF-α水平的相关性。结果:与睡眠正常组比较,睡眠障碍组H-Y分级、ADL评分及血浆TNF-α水平均显著升高(P<0.05)。血浆TNF-α水平与H-Y分级、ADL评分和PDSS-2评分均呈正相关关系(均P<0.05)。结论:帕金森病合并睡眠障碍患者的病情较睡眠正常者严重,生活自理能力亦较差,可能与血浆TNF-α水平增高有关。Objective：To investigate the clinical features of Parkinson’s disease combined with sleep disorder and its association with plasma tumor necrosis factor-α（TNF-α）level.Methods：From January 2014 to December 2015,99 patients with Parkinson’s disease were enrolled,and divided into sleep disorder group（n=59）and normal sleep group（n=40）.The severity of Parkinson’s disease was evaluated by Hoehn-Yahr grade（H-Y grade）.The plasma TNF-αlevel was measured by enzyme linked immunosorbent assay（ELISA）.The activity of daily living（ADL）score was compared between the two groups.The correlation of H-Y grade,sleep quality PDSS-2 score,ADL score and plasma TNF-αlevel was analyzed by Pearson linear correlation analysis.Results：The H-Y grade,ADL score and plasma TNF-αlevel were significantly higher in sleep disorder group than those in normal sleep group（P〈0.05）.The plasma TNF-αlevel was positively correlated with H-Y grade,ADL score and PDSS-2 score（P〈0.05）.Conclusion：Patients with Parkinson’s disease combined with sleep disorder had more severe condition and poorer self-care ability than normal sleep controls.It might associated with elevated plasma TNF-αlevel.
