摘 要:目的:探讨深圳地区城市居民腰围与血清胰岛素水平的关系。方法:采用横断面调查,对2008—2017年于深圳市南山区人民医院体检和内分泌门诊就诊的894例成年人进行口服葡萄糖耐量试验(OGTT)。收集研究对象的性别、年龄、体重、腰围、体重指数(Ibm)、舒张压(DBP)、收缩压(SBP)等一般资料,检测空腹血糖(FBG)、餐后2h血糖、空腹血清胰岛素水平、餐后2h血清胰岛素水平。采用单因素线性回归及多元线性回归法分析腰围与血清胰岛素水平的关系。结果:单因素线性回归分析显示,腰围、年龄、体重、Ibm、SBP、DBP、餐后2h血糖可能与空腹高胰岛素血症有关(均P<0.05)。校正性别、年龄、体重、Ibm、DBP、SBP、餐后2h血糖等因素后,多元线性回归分析显示:腰围是影响空腹血清胰岛素水平的独立危险因素(β=0.10,95%CI:0.04~0.17,P<0.01)。腰围对餐后2h血清胰岛素水平无明显影响(P>0.05)。结论:腰围对空腹血清胰岛素水平的影响较餐后2h血清胰岛素水平的影响更大,是空腹高胰岛素血症的危险因素。Objective:To study the association between waist circumference and serum insulin in urban residents in Shenzhen.Methods:By a cross-sectional study,894 subjects were selected from medical check and endocrinology clinics in Nanshan hospital of Shenzhen from 2008 to 2017,following 75 g oral glucose tolerance test(OGTT).The data were collected,including age,weight,waist circumference,body mass index,diastolic blood pressure(DBP),systolic blood pressure(SBP)and fasting blood glucose(FBG).The levels of postprandial 2-hour blood glucose,fasting insulin,and postprandial 2-hour insulin were detected.The association between waist circumference and serum insulin was analyzed by univariate linear regression and multiple linear regression analysis.Results:Univariate linear regression analysis showed that the waist circumference,age,body weight,body mass index,SBP,DBP and postprandial 2-hour blood glucose might be associated with fasting hyperinsulinemia(P〈0.05).After adjusting for confounding factors,multiple linear regression analysis revealed that waist circumference was an independent risk factor influencing fasting serum insulin level(β=0.10,95%CI:0.04-0.17,P〈0.01).Waist circumference had no obvious effect on postprandial 2-hour insulin level(P〉0.05).Conclusion:Waist circumference had a greater impact on fasting serum insulin level compared with postprandial 2-hour serum insulin level,and was an independent risk factor for fasting insulin level.
