1例替罗非班致极重度血小板减少症的案例分析

摘　　要：目的探讨替罗非班诱导极重度血小板减少症的特点、诊治原则及药学监护。方法临床药师通过回顾国内外关于替罗非班致血小板减少的文献,对此例患者的临床表现、诊疗策略及药学监护进行分析。结果替罗非班可在用药数小时内引起血小板的急剧下降,停用药物并予相应处理后,此例患者血小板计数在3日后恢复至正常范围,未出现严重出血反应。结论对经皮冠状动脉介入治疗(PCI)围术期出现血小板减少者,应根据患者病情、用药史及血小板减少特点,准确判断导致血小板减少的原因,并及时予以正确处理。临床药师应关注替罗非班引起的血小板减少症,治疗期间加强血小板计数监测和出血反应的监护。OBJECTIVE To investigate the features,diagnosis and treatment principles,and pharmaceutical care of profound thrombocytopenia caused by tirofiban. METHODS By reviewing the literatures about tirofiban-induced thrombocytopenia,the clinical manifestations,the diagnostic and therapeutic strategies and the pharmaceutical care of this case were analyzed by clinical pharmacist.RESULTS Tirofiban could cause a sharp drop in platelet within hours of administration. The platelet count in this patient was restored to normal range after 3 days by discontinuing the drug and treating it accordingly,and serious bleeding was not observed.CONCLUSION For patients with thrombocytopenia during PCI perioperative period,we should figure out the cause of thrombocytopenia exactly,and treat it timely and correctly basing on patients ＇ill conditions,drug histories and the features of thrombocytopenia. Clinical pharmacists should pay attention to the possible thrombocytopenia induced by tirofiban,and strengthen the monitoring of platelet count and the bleeding events.
