叶酸、维生素B_6、甲钴铵联合门冬胰岛素治疗GDM合并HHcy患者的效果及对围产儿并发症的影响

摘　　要：目的探讨叶酸、维生素B_6、甲钴铵联合门冬胰岛素治疗妊娠糖尿病(GDM)合并高同型半胱氨酸血症(HHcy)患者的效果及对围产儿并发症的影响。方法以2015年7月至2017年11月我院收治的88例GDM合并HHcy患者作为研究对象,按照病历号奇偶数分为对照组和观察组,各44例。对照组给予基础治疗+门冬胰岛素治疗,观察组在此基础上给予叶酸、维生素B_6和甲钴铵联合治疗,比较两组患者治疗效果和围产儿并发症发生情况。结果治疗后,观察组的Hcy、FBG、2 h PG水平均低于对照组(P<0.05);观察组围产儿并发症总发生率为4.55%,明显低于对照组的20.45%(P<0.05)。治疗1个月后,观察组IL-2、IL-6水平均低于对照组(P<0.05)。结论针对GDM合并HHcy患者,叶酸、维生素B_6、甲钴铵联合门冬胰岛素治疗效果显著,值得进一步推广。Objective To investigate the effect of folic acid, vitamin B_6, methylcobalamin combined with insulin aspart in the treatment of GDM with HHcy and its effect on perinatal complications. Methods A total of 88 patients with GDM and HHcy from July 2015 to November 2017 admitted in our hospital were selected and divided into control group and observation group according to the parity number of medical record, with 44 cases in each group. The control group was treated with basic treatment ＋insulin aspart, while the observation group was treated with folic acid, vitamin B_6 and methylcobalamin on the basis of the control group, the treatment effects and the incidences of perinatal complications in the two groups were compared. Results After treatment, the levels of Hcy, FBG and 2 h PG in the observation group were lower than those in the control group（P〈0.05）; the total incidence of perinatal complications in the observation group was 4.55%, which was significantly lower than 20.45% of the control group（P〈0.05）. One month after treatment, the levels of IL-2 and IL-6 in the observation group were lower than those in the control group（P〈0.05）. Conclusion For patients with GDM combined with HHcy,folic acid, vitamin B_6, methylcobalamin combined with insulin aspart is effective and worthy of further promotion.
