摘 要:目的探讨CT引导带钩钢丝(hookwire)应用肺部小结节术前定位的安全性与疗效。方法对2015年2月-2018年3月我院心胸外科收治的36例患者,术前均使用CT引导下hookwire定位肺部小结节,操作结束后行VATS下肺部病变切除,根据术中冰冻病理结果决定下一步手术方式。结果肺部小结节直径4-23mm,采用CT引导下hookwire定位成功率达100%,平均定位时间25min,有2例患者定位后CT扫描时发现有少量气胸,1例患者在操作过程中穿刺针脱落。结论术前使用CT引导下hookwire定位肺部小病变的方法安全性高、准确,能提高VATS手术中肺部小结节切除的准确率。Objective To explore the feasibility and safety of CT-guided hookwire localization of pulmonary nodule in video-assisted thoracic surgery(VATS). Methods 36 patients of cardiothoracic surgery department were collected from February 2015 to March 2018. CT guided hookwire was used to locate the pulmonary nodules before the operation. VATS was conducted after the procedure. Results The pulmonary nodules were 4-23mm in diameter,and the success rate of hookwire localization under the guidance of CT was up to 100%,with an average localization time of 25minutes. 2 patients were found to have a small amount of pneumothorax after localization in the CT scan,and 1 patient was found to have lost the puncture needle during the operation. Conclusion The method of CT guided hookwire to locate the pulmonary nodules before operation has high safety and accuracy,which can improve the accuracy of pulmonary nodules resection in VATS surgery.
