PFNA与解剖锁定钢板治疗不稳定性股骨粗隆间骨折的疗效分析

摘　　要：目的分析防旋股骨近端髓内钉(PFNA)与解剖锁定钢板治疗不稳定性股骨粗隆间骨折的疗效。方法 64例不稳定性股骨粗隆间骨折患者,随机分为对照组与观察组,每组32例。对照组患者给予解剖锁定钢板治疗,观察组患者给予PFNA治疗。比较两组患者手术时间、出血量、骨折愈合时间、并发症发生情况及髋关节功能情况。结果观察组患者手术时间、出血量、骨折愈合时间均少于对照组,差异具有统计学意义(P<0.05)。观察组患者并发症发生率为3.13%,低于对照组的21.88%,差异具有统计学意义(P<0.05)。观察组患者髋关节功能优良率为93.75%,高于对照组的75.00%,差异具有统计学意义(P<0.05)。结论给予不稳定性股骨粗隆间骨折患者PFNA治疗,能减少患者手术时间,促进骨折恢复,减少并发症的发生,促进髋关节功能的恢复,具有借鉴价值。Objective To analyze the efficacy of proximal femoral nail anti-rotation （PFNA） and anatomical locking plate in the treatment of unstable intertrochanteric fracture of femur. Methods A total of 64 patients with unstable intertroehanteric fracture of femur were randomly divided into control group and observation group, 32 cases in each group. Patients in the control group were treated with anatomical locking plate, and patients in the observation group were treated with PFNA. The operative time, hemorrhage volume, fracture healing time, complications and hip joint function between the two groups were compared. Results The operative time, hemorrhage volume and fracture healing time of the observation group were significantly less than those of the control group, the differences were statistically significant （P〈0.05）. The incidence of complications in the observation group was 3.13%, which was significantly lower than 21.88% in the control group, the difference was statistically significant （P〈0.05）. The excellent and good rate of hip joint function in the observation group was 93,75%, which was significantly higher than 75.00% in the control group, the difference was statistically significant （P〈0.05）. Conclusion The implementation of PFNA in the treatment of unstable intertrochanteric fracture of femur can reduce the operative time, promote fracture recovery, reduce the incidence of complications and promote the recovery of hip joint function, it has reference value.
