摘 要:目的探讨同步使用液体法和固体法培养在解脲支原体(Uu)和人型支原体(Mh)检测的临床价值,提高生殖道支原体检测的准确性及时效性。方法 202份来源疑似泌尿生殖道感染的生殖道标本同步做Uu、Mh的液体培养和固体培养,观察并记录液体法和固体法培养结果 ,用统计学方法评价两种方法的阳性率差异。结果 202份生殖道标本液体培养检出Uu阳性84份,阳性率41.6%,检出Mh阳性35份,阳性率17.3%;固体法培养检出Uu阳性61份,阳性率为30.2%,检出Mh阳性20份,阳性率为9.9%;两种培养法Uu和Mh阳性率差异均具有统计学意义(P<0.05)。23份液体培养阳性、固体培养阴性的再转种固体培养基,有5份可见典型Uu菌落,其中3份男性尿道标本,2份女性宫颈分泌物标本。结论液体法能一步完成支原体培养、鉴定和药敏试验,但结果易受杂菌影响造成假阳性,不宜单独作为诊断生殖道支原体感染实验室依据;固体法培养可见典型支原体菌落形态,能排除液体法的假阳性。两种培养法同步联合检测既能缩短支原体检测周期,又能保证检测结果的准确性。Objective To investigate the clinical value of simultaneous liquid and solid culture in the detection of ureaplasma urealyticum (Uu) and mycoplasma hominis (Mh), in order to improve the accuracy and timeliness of mycoplasma detection in genital tract. Methods A total of 202 genital tract specimens from suspected genitourinary tract infections were cultured Uu and Mh in liquid and solid phases simultaneously. The results of liquid culture and solid culture were observed and recorded. The difference of positive rate between the two methods was evaluated by statistical method. Results In 202 genital tract specimens, 84 specimens were Uu positive, the positive rate was 41.6%, 35 specimens were Mh positive, the positive rate was 17.3%. 61 positive Uu samples were detected by solid culture method, the positive rate was 30.2%, and 20 positive Mh samples were detected, the positive rate was 9.9%. The difference in Uu and Mh positive rate between the two methods was statistically significant (P〈0.05). 23 liquid culture positive and solid culture negative reintroduced solid media showed typical Uu colonies in five of them, including three male urethral specimens and two female cervical secretions. Conclusion Liquid method can accomplish mycoplasma culture, identification and drug susceptibility test in one step, but the result is easily affected by the mixed bacteria, and can not be used as a laboratory basis for diagnosis of genital tract mycoplasma infection. The morphology of typical mycoplasma colonies can be seen in solid culture, and the false positive of liquid method can be ruled out. The simultaneous detection of mycoplasma by two culture methods can not only shorten the detection period of mycoplasma, but also ensure the accuracy of detection results.
