摘 要:目的:探讨甲状腺球蛋白抗体(TGAb)、促甲状腺激素受体抗体(TRAb)、甲状腺微粒体抗(TMAb)和甲状腺过氧化物酶抗体(TPOAb)联合检测在妊娠期甲减的诊断作用和意义。方法:选取2015年1月~2017年1月本院门诊产检并随诊的120例妊娠期甲减患者为实验组,选取同期健康体检女性妊娠患者50例为对照组。收集患者血清,并检测血清中TGAb、TRAb、TMAb和TPOAb水平变化进行统计分析。结果:与对照组妊娠早期相比,其他几组患者血清内TSH浓度有明显增高,妊娠中期和晚期患者血清内FT3和FT4含量明显降低,差异具有统计学意义(P <0. 05);观察组妊娠中期和妊娠晚期患者血清内TSH有明显增高,FT3、FT4浓度明显降低,差异具有统计学意义(P <0. 05)。TGAb、TRAb、TMAb和TPOAb浓度有明显增高,差异具有统计学意义(P <0. 05)。结论:妊娠期甲减患者血清中TGAb、TRAb、TMAb和TPOAb含量呈过度表达状态; TGAb、TRAb、TMAb和TPOAb的联合检测在妊娠期甲减的诊断中具有重要的临床意义。Objective: To investigate the diagnostic effects of thyroglobulin antibody( TGAb),thyrotropin receptor antibody( TRAb),thyroid microsomal resistance( TMAb) and thyroid peroxidase antibody( TPOAb) in the diagnosis of hypothyroidism during pregnancy And meaning. Methods: A total of 120 patients with gestational hypothyroidism were enrolled in this study from January 2015 to January 2011. Fifty healthy pregnant women were selected into the control group. Serum levels of TGAb,TRAb,TMAb and TPOAb in serum were collected and analyzed statistically. Results: Compared with the control group,the serum TSH concentration was significantly higher in the other groups,and the levels of FT3 and FT4 in the serum of middle and late pregnancy were significantly decreased,the difference was statistically significant( P〈0.05). In the observation group,serum TSH was significantly higher in the middle and third trimester of pregnancy,and the concentrations of FT3 and FT4 were significantly lower( P〈0. 05). TGAb,TRAb,TMAb and TPOAb concentrations were significantly increased,the difference was statistically significant( P〈0.05). Conclusion: The levels of TGAb,TRAb,TMAb and TPOAb in serum of pregnant patients with hypothyroidism were overexpressed. TGAb,TRAb,TMAb and TPOAb have important clinical significance in the diagnosis of hypothyroidism during pregnancy.
