血清神经调节蛋白4水平与2型糖尿病患者体重的关系

摘　　要：目的探讨血清神经调节蛋白4(NRG4)水平与2型糖尿病(T2DM)患者体重的关系。方法 T2DM患者157例。其中,体重正常者47例(A组,BMI<24kg/m2),超重者78例(B组,24kg/m2≤BMI<28kg/m2),肥胖者32例(C组,BMI≥28kg/m2)。采用ELISA法检测三组血清NRG4浓度,同时测定其他代谢指标。结果 A组血清NRG4水平高于B、C组[(29.54±9.56)pg/ml vs.(9.76±4.42)pg/ml、(4.50±2.00)pg/ml](P<0.05),B组亦高于C组(P<0.05)。Pearson相关分析显示,血清NRG4水平与HOMA-IR、BMI、腰围、臀围、腰臀比、FBG、TG和尿酸呈负相关(r=-0.581、-0.579、-0.461、-0.357、-0.317、-0.202、-0.161和-0.159,P<0.05),与HDL-C呈正相关(r=0.241,P<0.01)。多元线性逐步回归分析显示,HOMA-IR、BMI、TG和FBG是血清NRG4水平的独立影响因素(P<0.05)。结论超重及肥胖T2DM患者的血清NRG4水平是降低的;血清NRG4可能参与了糖尿病患者胰岛素抵抗的发生,并与T2DM患者超重及肥胖的发生有关。Objective To investigate the relationship of serum neuregulin 4（NRG4）level and body weight in the patients with type 2 diabetes mellitus（T2DM）.Methods A total of 157 patients with T2 DM was divided into three groups of A（BMI〈24 kg/m2,47 cases）,B（24 kg/m2≤BMI〈28 kg/m2,78 cases）and C（BMI≥28 kg/m2,32 cases）.Serum NRG4 concentration was detected by ELISA.The other metabolic indicators were determined as well.Results Serum level of NRG4 in group A was higher than that in groups of B and C[（29.54±9.56）pg/ml vs.（9.76±4.42）pg/ml and（4.50±2.00）pg/ml]（P〈0.05）,which was higher in group B than that in group C（P〈0.05）.Pearson correlation analysis showed that serum level of NRG4 was negatively correlated with HOMA-IR,BMI,wastline,hipline,ratio of wast to hip,FBG,TG and uric acid（r=-0.581,-0.579,-0.461,-0.357,-0.317,-0.202,-0.161 and-0.159,P〈0.05）and positively correlated with HDL-C（r=0.241,P〈0.01）.Multivariate linear stepwise regression analysis showed that HOMA-IR,BMI,TG and FBG were the independent factors influencing serum NRG4 level（P〈0.05）.Conclusion Serum level of NRG4 is decreased in T2DM patients with overweight and obesity,which may be involved in the development of insulin resistance and overweight in T2DM patients.
