摘 要:目的:探讨三七茜草复方有效组分(SQ)宫内缓释系统调控大鼠子宫内膜出凝血机制。方法:选定32只SD雌性大鼠,随机分为SQ-IUD组、IUD模型组、缓释材料(LR)对照组、空白对照组,观察大鼠子宫大体形态学、子宫内膜形态学,ELISA检测宫腔冲洗液内前列环素(PGI2)、血栓烷(TXA2)、纤溶酶原激活物抑制剂(PAI-1)、组织型纤溶酶原激活剂(t-PA)的含量。结果:在子宫大体形态学方面,SQ-IUD组充血水肿和渗出粘连程度较IUD模型组明显减轻;在子宫内膜形态学方面,SQ-IUD组上皮细胞脱落、腺体扩张、间质水肿、炎性细胞浸润程度较IUD模型组明显减轻;在PAI-1、t-PA方面,SQ-IUD组PAI-1的含量较IUD模型组明显升高,SQIUD组、IUD模型组PAI-1的含量分别为(205. 45±16. 03) pg/m L、(177. 22±22. 08) pg/m L(P 〈0. 05),SQ-IUD组t-PA的含量较IUD模型组明显降低,SQ-IUD组、IUD模型组t-PA的含量分别为(3. 20±0. 29) ng/m L、(3. 61±0. 47) ng/m L(P 〈0. 05);在TXA2、PGI2方面,SQ-IUD组二者的含量较IUD模型组均明显升高,SQIUD组、IUD模型组TXA2的含量分别为:(260. 88±23. 40) pg/m L、(228. 68±31. 31) pg/m L(P 〈0. 05),SQ-IUD组、IUD模型组的PGI2含量分别为:(235. 25±23. 59) pg/m L、(200. 88±22. 90) pg/m L(P 〈0. 05);在TXA2/PGI2方面,SQ-IUD组的比值较IUD模型组略低,SQ-IUD组、IUD模型组的TXA2/PGI2比值含量分别为:(1. 12±0. 15)、(1. 14±0. 14)(P〉 0. 05)。结论:三七茜草缓释系统可明显减轻IUD放置后子宫炎症反应,改善子宫内膜形态;三七复方有效成分可能通过调节t-PA、PAI-1和TXA2、PGI2平衡而起到防治IUD出血不良反应。Objective:To explore the effective components of notoginseng madder compound( SQ) of Levonorgestrel-releasing intrauterine system on endometrial morphology of rats and blood coagulation mechanism. Methods:Thirty-two SD female rats were randomly divided into SQ-IUD group,IUD model group,LR control group and blank control group.We observed rats' uterus morphology and endometrial morphology. ELISA was used to detect the contents of propion(PGI2),thrombutane(TXA2),plasminogen activator inhibitor(PAI-1) and tissue plasminogen activator(t-pa) in the uterine washing fluid. Results:In the general morphology of the uterus,the level of congestion,edema and adhesion of SQ-IUD group were obviously lower than those of the IUD model group. In terms of endometrial morphology,the level of epithelial cell shedding,glands expansion,interstitial edema and inflammatory cells infiltration of SQ-IUD group were obviously lower than those of IUD model group. In terms of PAI-1 and t-PA,the contents of PAI-1 of SQ-IUD group were obviously higher than those of IUD model. The contents of PAI-1 of SQ-IUD group and IUD model were(205. 45 ± 16. 03)pg/m L,(177. 22 ± 22. 08)pg/m L respectively(P〈0. 05). The content of t-PA of SQ-IUD group was obviously lower than that of IUD model. The contents of t-PA of SQ-IUD group and IUD model were(3. 20 ±0. 29) ng/m L and(3. 61 ± 0. 47) ng/m L respectively( P〈0. 05). In terms of TXA2 and PGI2,the contents of TXA2 and PGI2 of SQ-IUD group were obviously higher than those of IUD model.
